2型糖尿病患者尿酸水平与肾功能关系 　　【摘要】 目的：研究2型糖尿病患者血尿酸水平与估算肾小球滤过率（eGFR）的相关关系，探讨血尿酸变化对肾功能的影响。方法：采用横断面回顾性研究方法，选取2016年1月-2016年8月于本院住院的323例2型糖尿病患者为研究对象，建立线性回归模型分析患者血尿酸水平与肾功能变化的关系，并分析肾功能进展的危险因素。结果：323例2型糖尿病患者中男186例，女137例，年龄（56.9±13.1）岁， 　　25.4%患者合并高尿酸血症，eGFR90 mL/（min?1.73 m2）组血尿酸水平及高尿酸血症比例明显升高（P0.05）。方差分析显示尿酸水平高组其CKD（CKD3a-CKD5期）的发病率明显高于低尿酸水平组（P0.05）。调整年龄、性别、糖尿病等因素后，多元线性回归分析结果显示：血尿酸基线值每增高100 μmol/L，eGFR值下降7.47 mL/（min?1.73 m2），95%CI为（-9.23，-5.72），P0.05。Logistic回归分析显示年龄、性别、尿酸、合并高血压是2型糖尿病患者肾功能进展的独立危险因素。结论：血尿酸是2型糖尿病患者肾功能进展的独立预后影响因素，临床上应重视并合理地控制尿酸水平以延缓肾功能减退。 　　【关键词】 糖尿病； 尿酸； 肾小球滤过率； 危险因素 　　【Abstract】 Objective：To analyze the relationship of serum uric acid level and estimated glomerular filtration rate （eGFR） of type 2 diabetic patients，and to explore the influence of serum uric acid change on renal function. Method：Based on retrospectively crosssectional study method，323 type 2 diabetic patients were enrolled in this study who were hospitalized from January 2016 to August 2016.A linear regression model was established to analyze the relationship between serum uric acid level and renal function，and analyzed the risk factors of the progression of renal function.Result：In the 323 cases，there were 186 male and 137 female，（56.9±13.1） years old， and 25.4% of patients had hyperuricemia.The serum uric acid level and the proportion of hyperuricemia were significantly increased in the eGFR90 mL/（min?1.73 m2） group（P0.05）.And ANOVA showed that compared with the lower uric acid group，the incidence of CKD （CKD3a-CKD5） was significantly higher in the high uric acid group（P0.05）.Multiple regression analysis showed that after adjustment for age，gender，diabetes and so on，each 100 μmol/L higher of uric acid was 7.47 mL/（min?1.73 m2） decreased in eGFR，95%CI（-9.25，-5.72），P0.05.Logistic regression analysis showed that age，sex，uric acid and hypertension were independent risk factors for progression of renal function in patients with type 2 diabetes.Conclusion：Serum uric acid is an independent prognostic
