BI―RADS对乳腺钼靶体检中钙化灶处理作用 　　[摘要] 目的 探讨乳腺影像报告及数据系统（breast imaging-report and data system，BI-RADS）在乳腺钼靶体检中钙化灶处理的作用。 方法 复阅2013年586例乳腺钼靶体检中发现224例钙化，按照BI-RADS对钙化进行描述，并进行BI-RADS分类，对良性钙化BI-RADS 2类进行1年半随诊，未发现乳腺癌，对可疑恶性钙化BI-RADS 4类及恶性钙化BI-RADS 5类利用数字化乳腺钼靶三维立体导丝定位技术定位后行外科区段切除，并与病理结果进行对照研究。 结果 218例典型良性钙化BI-RADS 2类进行1年半随诊，未发现乳腺癌；可疑恶性钙化BI-RADS 4类 2例及高度恶性可能钙化BI-RADS 5类 4例用数字化乳腺X线三维立体导丝定位技术定位后行外科区段切除，病理结果显示4例为乳腺癌。 结论 BI-RADS分类对健康人群乳腺钼靶体检中发现钙化灶的处理有积极作用。 　　[关键词] 乳腺；数字化乳腺钼靶；钙化；BI-RADS 　　[中图分类号] R737.9；R655.8 [文献标识码] B [文章编号] 1673-9701（2015）31-0099-04 　　Effect of BI-RADS on calcification management during mammary gland molybdenum target examination 　　HE Xiao DUAN Xueyan FENG Gang 　　Department of Radiology， Xiamen Third Hospital， Xiamen 361100， China 　　[Abstract] Objective To discuss the effect of breast imaging-report and data system（BI-RADS） in calcification management during mammary gland molybdenum target examination. Methods A total of 224 cases of calcification among 586 cases of mammary gland molybdenum target examination were reviewed and the calcifications were described and classified according to BI-RADS. For benign calcifications of BI-RADS class 2， a follow-up lasting 1.5 years was conducted and no breast cancer was observed. Suspicious malignant calcifications of BI-RADS class 4 and malignant calcifications of BI-RADS class 5 were surgically resected after localization by digital mammary gland molybdenum target 3D godet positioning technology and then compared with pathological results. Results No breast cancer was observed among the 218 benign calcifications of BI-RADS class 2 during the 1.5 years follow-up. Two cases of suspicious malignant calcifications of BI-RADS class 4 and 4 cases of malignant calcifications of BI-RADS class 5 were surgically resected after localization by digital mammary gland molybdenum target 3D godet positioning technology and the pathological results showed 4 cases of breast cancer.
