BISAP评分对急性胰腺炎严重程度预测价值研究 　　【摘要】目的：探讨急性胰腺炎严重程度床边指数评分（BISAP）在预测急性胰腺炎（AP）严重程度中的价值。方法：运用BISAP、APACHE II、Rason对61例AP患者进行回顾性评分，比较轻症组与重症组、器官衰竭组与非器官衰竭组的评分差异，比较各评分高分组与低分组之闻重症急性胰腺炎（SAP）发生率、器官衰竭发生率差异。结果：重症组与轻症组、器官衰竭组与非器官衰竭组的APACHEII、Rason、BISAP差异均有统计学意义（P0.01）。BISAP，APACHEII，Rason各自高分组和低分组患者SAP、器官衰竭发生率差异均有统计学意义（P0.01）。结论：BISAP简便易行，对于判断AP严重程度及器官衰竭具有重要参考意义。 　　【关键词】急性胰腺炎；急性胰腺炎严重程度床边指数评分；急性病生理学和长期健康评价；Rason评分 　　Values of BISAP Scoring System in Predicting the severity of Acute Pancreatitis 　　LUO Xu，YE Jing Song，LIANG Zhao 　　people’s hospital of new district LongHua ShenZheng 518109，China 　　【Abstract】 Objective： To asses the values of Bedside Index of Severe Acute Pancreatitis（BISAP） scoring system in predicting the severity of acute pancreatitis（AP）. Methods： 61 patients with AP were retrospectively studied with three scoring systems： BISAP， APACHE Ⅱand Rason. Differences of scores between the mild group and the severe group， the organ failure group and the non-organ failure group were compared. Differences in severe acute pancreatitis（SAP）rates，organ failure rates between the high score group and the low score group were also compared.Results： The mean scores of APACHE Ⅱ，Rason and BISAP were significantly different between severe and mild pancreatitis and between the organ failure group and the non-organ failure group（P0.01）.For these scoring systems， differences in the incidence of SAP，organ failure rates were statistically significant between the high score group and the low score group（P0.01）.Conclusion： BISAP was a simple and convenient scoring system， could be used as a important reference for the evaluation of severity and organ failure of AP. 　　【Key words】acute pancreatitis； BISAP； APACHE Ⅱ； Rason 　　急性胰腺炎（acute pancreatitis，AP）是胰腺炎的急性炎症过程，20%-30%病人会发展成重症胰腺炎。一个客观、可靠的风险评估系统用于预测AP的严重程度及预后，在胰腺炎的治疗过程中有着相当重要的临床意义。 　　我们通过对本院92例急性胰腺炎患者进行急性胰腺炎严重程度床边指数评分（BISAP），并与Ranson 评分系统和急性生理学及慢性健康状况（acute physiology and chronic health evaluationⅡ， APACHE Ⅱ）评分系统进行比较，以判断BISAP评分对急性胰腺炎严重程度
