AVE―763B尿液沉渣分析仪联合检测在肾病诊断中应用 　　[摘要] 目的 探讨尿液中红细胞的参数差异及白细胞、上皮细胞、管型等项目在鉴别诊断肾病中的运用价值。 方法 利用国产AVE-763B尿液沉渣分析仪，对本院2014年1月～2015年5月住院患者与门诊患者456例血尿结果着重进行红细胞参数指标统计分析，包括畸形红细胞率（Rs）、尿红细胞宽度分布（Rd）、尿红细胞形态分布（Rv）、尿红细胞色度分布（Rc）；并对这些标本中的白细胞、上皮细胞、管型等项目进行分析，进而探讨与肾病的关系。结果 Rs70%、Rv80%时，非肾性血尿与临床的符合率达95%。肾炎患者中一般不出现白细胞，小圆上皮细胞反映出肾小管信息，急性肾小球肾炎时常见；管型的出现，尤其是红细胞管型，多提示肾实质病变。 结论 AVE-763B尿液沉渣分析仪联合检测红细胞参数、白细胞、上皮细胞、管型等项目对鉴别肾性出血和非肾性出血具有重要的临床价值。 　　[关键词] 尿液；红细胞参数；白细胞；管型；上皮细胞；肾病 　　[中图分类号] R446.12 [文献标识码] B [文章编号] 1673-9701（2016）10-0122-03 　　[Abstract] Objective To investigate the parameter differences of urine erythrocyte and the application value leukocyte， epithelial cells and cast in the differential diagnosis of nephropathy. Methods The AVE-763B urine sediment analyzer made in China was used to statistically analyze the erythrocyte parameter indices of the urine blood results of 456 inpatients and outpatients of our hospital from January 2014 to May 2015， including abnormal erythrocyte rate （Rs）， urine erythrocyte width distribution（Rd）， urine erythrocyte species distribution（Rv） and urine erythrocyte chroma indexing （Rc）. And items of these specimens such as leukocyte， epithelial cells and cast were analyzed in order to explore their relationship to nephropathy. Results With Rs70% and Rv80%， the accordance rate of non-renal hematuria to clinical practice reached up to 95%. Leukocyte was rarely seen in the patients with nephritis and renal tubule information was reflected by small round epithelial cells， which was commonly seen in acute glomerulonephritis. Renal parenchyma lesion was suggested by the occurrence of cast， especially erythrocyte. Conclusion The AVE-763B urine sediment analyzer combination detection of erythrocyte parameters， leukocyte， epithelial cells and cast have important clinical value in the differentiation of renal bleeding and non-renal bleeding. 　　[Key words] Urine； Erythrocyte parameters； Leukocytes； Cast； Epithelial cells； Nephropathy 　　肾病包含有肾小球肾炎、肾盂肾炎、IgA肾病、糖尿病肾病、肾病综合征等等。
