主动呼吸控制系统在肺癌放疗中的应用 　　[摘要] 目的：探讨主动呼吸控制(active breathing coordination，ABC)技术在肺癌精确放疗中的应用。方法：选取2006~2007年间来我科的12例肺癌患者，使用ABC技术联合三维适形放疗技术治疗，CT定位扫描时分别采集ABC和自由呼吸(free breath，FB)两套图像，计算比较两者胸壁动度、膈肌动度、大体靶区（gross tumor volume，GTV）、临床靶区（clinnical target volume，CTV）、计划靶区(planning target volume，PTV)及双肺V20、Dmean。结果：ABC状态下胸壁动度、膈肌动度、GTV、CTV、PTV及双肺V20、Dmean比FB状态下均要小，且差异有统计学意义（P0.05）。结论：在肺癌的精确放射治疗中，ABC系统可以有效的降低呼吸运动对治疗的影响，提高放疗的精确性，减少副反应。 　　[关键词] 肺癌；呼吸控制ABC；精确放疗 　　[中图分类号]R734.2 [文献标识码]A [文章编号]1673-7210（2007）10（b）-020-02 　　 　　The use of active breathing control in radiotherapy of lung cancer 　　ZHOU Gui-e，YANG Chao-feng，LI Li-ping，LI Shan-bai，YANG Hai-ming 　　(Department of Radiotherapy, Cancer Hospital of Guangxi Zhuang Autonomous Region, Nanning 530021, China) 　　[Abstract] Objective：To discuss the application in accurate radio therapy of lung cancer using the active breathing control (ABC).Methods：Choose 12 cases of lung cancer patients between 2006 and 2007，using of active breathing control in radiotherapy as a treatment technology.Each patient received computed tomography(CT) scans under two respiration：free breath(FB)and active breath-hold.Comparisons differences in the chest wall and diaphragm mobility，GTV， CTV，PTV，V20 and Dmean between ABC and FB plansy. Results：Differences in the chest wall and diaphragm mobility，GTV，CTV，PTV，V20 and Dmean between ABC and FB plans were significant（P0.05）.Conclusion：In the precise radiotherapy of lung cancer，the ABC system reduced the influence of breathing，improved the precision of the radiotherapy and reduced the toxicity. 　　[Key words] Lung Cancer；Active Breathing Control；Precision Radiotherapy 　　 　　近几年来，放疗技术由传统的二维常规放疗发展到精确放疗，它的应用减少了对邻近靶区正常组织器官的放射损伤，从而提高了肿瘤区照射剂量、局部控制率和患者的生活质量。精确放疗在肺部肿瘤的治疗中发挥着重要的作用[1]，并取得了较好的效果，但是肺部肿瘤易受到呼吸运动的影响而发生位移。因此控制呼吸运动，增加肺部肿瘤放疗剂量，成为国内外研究的热点[2]。本文通过对12例非小细胞肺癌患者使用主动呼吸控制系统 (active breathing control，ABC)做精确放疗，并与FB状态下做比较，现报道如下。 　　 　　1 资料与方法 　　 　　1.1 一般资料 　　选择2006年7月～2007年7月来我院经病理或细胞学确诊的周围型非小细胞肺癌的初治患者12例，其中男7例，女5例。年龄
