内镜下切除结肠巨大息肉临床分析 　　[摘要] 目的 分析内镜下切除结肠巨大息肉的临床效果。方法 方便选取来该院就医的经肠镜检查发现的76例直径 3 cm大肠息肉进行内镜下治疗患者（2013年1月―2015年12月）作为该次研究的对象，针对其采用内镜下切除手术治疗，探究手术效果差异性。结果 101处大息肉均成功切除，在切除过程均未出现穿孔，其中5例出现术后大出血，4例经内镜下止血保守治疗成功，另1例急诊外科手术腹腔镜下止血治疗，其中11例癌变息肉ESD完整切除，将切除标本送病理为顶部癌变，基底部及黏膜下未见癌浸润。术后情况较佳，且在术后随访3～12个月行肠镜复查，到目前复发1例。结论 内镜下切除结肠巨大息肉具有一定的应用价值，值得临床进一步推广以及应用。 　　[关键词] 内镜下；切除；结肠巨大息肉；临床分析 　　[中图分类号] R735 [文献标识码] A [文章编号] 1674-0742（2017）09（c）-0094-04 　　Clinical Analysis of Endoscopic Resection of Large Colorectal Polyps 　　ZHANG Li-min， ZHANG Jie， WANG Fu-hua， YANG Xiao-lan， LIN Chuan， ZENG Jin-rong， ZHU Jian-chen 　　Digestive System Department， Longyan Second Hospital， Longyan， Fujian Province， 364000 China 　　[Abstract] Objective To analyze the clinical effect of endoscopic resection of large colorectal polyps. Methods 76 cases of patients whose diameter 3 cm treated under the endoscope in our hospital from January 2013 to December 2015 were convenient selected as the research objects， and all patients were treated with endoscopic resection surgery， and the difference in the operation effect was studied. Results 101 large polyps were successfully resected and there was no perforation in the resection course， including 5 cases with postoperative hematorrhea， 4 cases with successful endoscopic hemostasis conservative treatment and 1 case with hemostasis treatment under the operational laparoscope in the department of emergency surgery， and the canceration polyps of 11 cases were completely resected， and the resection specimens were the top cancerization， and there were no cancer infiltration in basal part and under the mucous membrane， and the postoperative situation was better， and the patients were reexamined after 3～12 months follow-up， and 1 case had reoccurred so far.
