小剂量螺内酯治疗80岁以上高龄老人顽固性高血压疗效分析 　　【摘要】 目的：通过对高龄老人顽固性高血压患者加用小剂量螺内酯治疗，观察小剂量螺内酯对高龄老人顽固性高血压治疗的安全性及有效性。方法：选取2012年1月-2013年12月于本院干部病房住院的符合顽固性高血压诊断标准的80岁以上高龄老人42例，所有患者均不能耐受特殊检查及有创非药物治疗，在包括利尿剂在内的三联降压药治疗基础上，给予小剂量螺内酯20 mg/d，口服或鼻饲，共治疗1个月，观察收缩压及舒张压下降的情况。期间如果出现血压明显下降先调整其他降压药的应用，如果其他药物的剂量不能再减时，调整螺内酯的剂量；若血压无明显下降则不调整其他药物的种类及剂量。监测加用小剂量螺内酯治疗前后患者收缩压、舒张压、血清钾及血清肌酐变化情况；观察有无干咳、男性乳腺发育等不良反应发生。结果：治疗后所有患者收缩压、舒张压均有下降，与治疗前比较差异均有统计学意义（P0.05）。所有患者无干咳及男性乳房发育出现。结论：小剂量螺内酯对80岁以上高龄老人顽固性高血压治疗有效且安全。 　　【关键词】 螺内酯； 高龄老人； 顽固性高血压 　　Curative Effect Analysis of Spironolactone on People Over 80 Years Old with Resistant Hypertension/CHEN Yu-fei，YANG Ping，LI Yu.//Medical Innovation of China，2014，11（16）：060-063 　　【Abstract】 Objective：To add the low dose of spironolactone in the elderly patients with refractory hypertension， and observe the safety and efficacy of the small dose spironolactone in the treatment of elderly patients with refractory hypertension.Method： 42 cases of 80 years old refractory hypertension in January 2012 to December 2013 in our hospital were selected， and they were conformed to the diagnosis standard of resistant hypertension.
