探讨脐静脉导管封管可行性和效果 　　【摘要】 目的：探讨脐静脉导管封管的可行性和效果。方法：选取2013年1月-2015年2月本院新生儿科留置脐静脉导管的早产儿80例作为研究对象，将其进行随机分组，每组40例，研究组用生理盐水进行脉冲正压封管，而对照组用0.5 U/mL肝素钠盐水稀释液封管，在封管期间观察并记录两组导管留置时间、住院时间、封管前后血小板参数、封管前后凝血功能、脐静脉导管通畅度、脐静脉置管相关并发症发生情况等各指标，并对结果进行分析比较。结果：两组患儿导管留置时间和住院时间比较，差异均无统计学意义（P0.05）；封管前后两组血小板参数（PLT、PCT、PDW）、凝血功能（PTA、PTR、PT、INR、APTT、FIB、TT）比较，差异均无统计学意义（P0.05）。除此之外，两组患儿脐静脉导管通畅度情况比较，差异无统计学意义（P0.05），均没有明显堵塞情况发生，顺利完成输液；两组患儿脐静脉置管相关并发症发生情况大致相同，差异无统计学意义（P0.05）。结论：用生理盐水作为脐静脉导管封管液，能够维持导管的通畅度，不影响脐静脉导管的正常使用，且更符合新生儿的生理健康，经济、安全、具有临床应用价值，值得推广。 　　【关键词】 脐静脉导管； 封管； 肝素钠； 生理盐水； 　　【Abstract】 Objective：To investigate the feasibility and effect of umbilical vein catheter sealing.Method：80 premature infants in our hospital from February 2014 to October 2014 were randomly divided into two groups，40 cases in each group.The control group was treated with heparin sodium salt tube sealing，the study group was treated with Heparin Sodium Salt tube sealing.Observed and recorded during sealing tube catheter indwelling time，hospitalization time，sealing tube coagulant function before and after sealing tube，degree of umbilical vein catheter unobstructed，umbilical vein catheter related complications such as value of each index of two groups，the results were compared.Result：There was no significant difference between two groups of patients with catheter indwelling time and length of hospital stay，the platelet parameters（PLT，PDW，PCT），coagulation function（PTA，PTR，PT，INR，TT，FIB） of two groups were not significantly different（P0.05），except for this，there were no obvious differences in the patency of umbilical vein in two groups （P0.05）.Conclusion：The effect of normal saline as umbilical venous catheter sealing fluid is remarkable，it can maintain the patency of the catheter，not affect the normal use of umbilical vein catheters，and has more in line with the physiological health of newborns，it is with clinical application value and worthy of promotion. 　　【Key words】 Umbilical vein catheter； Sealing tube； Heparin Sodium； No
