无创正压通气治疗慢性阻塞性肺疾病急性加重期所致呼吸衰竭效果分析 　　[摘要] 目的 分析无创正压通气治疗慢性阻塞性肺疾病急性加重期所致呼吸衰竭的效果。 方法 将2012年1月～2013年6月来本院就诊的慢性阻塞性肺疾病急性加重期所致呼吸衰竭的患者随机分成两组，对照组34例，给予常规治疗，观察组38例，在此基础上进行无创正压通气。比较两组治疗前后呼吸困难基础指数（BDI）、生活质量（SGRQ）评分和住院时间；比较治疗前后两组的呼吸困难症状：呼吸频率、心率、动脉血气（PaO2、PaCO2）。结果 治疗7d后，观察组的BDI评分显著高于对照组，SGRQ评分显著低于对照组；观察组的住院时间显著短于对照组（P0.05）；治疗后第1、7天后，观察组的呼吸频率、心率和PaCO2显著低于对照组，PaO2显著高于对照组（P0.05）。 结论 无创正压通气治疗慢性阻塞性肺疾病急性加重期所致呼吸衰竭的效果好，患者的呼吸困难和生活质量能得到更好的改善。 　　[关键词] 无创正压通气；慢性阻塞性肺疾病；呼吸衰竭 　　[中图分类号] R714.253[文献标识码] A[文章编号] 1674-4721（2014）05（a）-0026-03 　　Effect analysis of non-invasive positive pressure ventilation in the treatment of respiratory failure caused by acute exacerbation of chronic obstructive pulmonary disease 　　ZHU Yun-que 　　Department of Internal Medicine，People′s Hospital of Heping County in Heyuan City of Guangdong Province，Heping 517200，China 　　[Abstract] Objective To analyze the effect of non-invasive positive pressure ventilation （NIPPV） in the treatment of respiratory failure caused by acute exacerbation of chronic obstructive pulmonary disease （COPD）. Methods Patients with COPD at acute exacerbation stage complicated with respiratory failure were randomly divided into the control group （n=34） and the observation group （n=38）.In the control group，regular treatment was provided，while in the observation group，on the basis of treatment in the control group，NIPPV was added.The basic dyspnea index （BDI），quality of life （SGRQ），and hospital stay before and after treatment were compared in both groups.Symptoms of dyspnea like respiratory rate （RR），heart rate （HR），and arterial blood gases including PaO2 and PaCO2 before and after therapy were also compared. Results After 7 days treatment，in the observation group，the BDI score was higher，SGRQ lower，and hospital stay shorter in comparison of those in the control group （P0.05）. after treatment the first day and seventh day，the RR，HR，and PaCO2 in the observation group was lower，and PaO2 was higher compared with those in the control group （P0.05）
