特异性免疫治疗对慢性荨麻疹患者临床疗效及其对血清白介素18水平影响 　　[摘要] 目的 探讨特异性免疫治疗对慢性荨麻疹患者临床疗效及其对血清白介素-18水平的影响。 方法 选择2008年9月～2010年10月在我院门诊就诊的慢性荨麻疹患者68例作为研究对象。随机将其分为观察组和对照组，每组各34例。对照组予以抗组胺药物10 mg，每日1次口服，连续服用3个月。观察组在对照组治疗基础上根据患者皮肤点刺过敏原试验及特异性IgE检测结果采用特异性免疫治疗，特异免疫治疗疗程为12个月。两组患者同时予以卡介菌多糖核酸注射液0.5 mg肌注，隔日1次，连续应用3个月。 结果 观察组总有效率为82.35%，对照组总有效率为58.82%，两组总有效率比较，差异有统计学意义（P ＜ 0.05）。治疗后观察组IL-18水平与治疗前比较差异有高度统计学意义（P ＜ 0.01），与对照组比较差异有高度统计学意义（P ＜ 0.01）。 结论 特异性免疫疗法能够下调IL-18，纠正慢性荨麻疹患者Th1/Th2细胞因子的失衡，疗效肯定。 　　[关键词] 慢性荨麻疹；特异性免疫；血清白介素-18 　　[中图分类号] R758.24 [文献标识码] A [文章编号] 1673-7210（2012）01（b）-037-02 　　 　　Clinical effect of specific immunotherapy on patients with chronic urticaria and their serum interleukin-18 　　LI Weixiong LIANG Shurong CAI Zhongqun ZHANG Guanping 　　Department of Dermatology, the Peoples Hospital in Yunfu City, Guangdong Province, Yunfu 527300, China 　　[Abstract] Objective To explore the clinical effect of specific immunotherapy on patients with chronic urticaria and their serum interleukin-18. Methods 68 patients with chronic urticaria from September 2008 to October 2010 in our hospital were selected as the subjects in this study and were randomly allocated into two groups, including observation group and control group, each of which had 34 cases. In control group, the patients were given antihistamine drug following oral administration with the dosage of 10 mg once a day for three month. In observation group, the patients were given allergen test with fast insertion on skin and specific immunotherapy for 12 months was performed to test specific IgE based on the treatment of control group.
