纤支镜肺泡灌洗联合抗生素治疗社区获得性肺炎临床价值 　　【摘要】目的：探究纤支镜肺泡灌洗联合抗生素治疗（Community acquired pneumonia，CAP）的临床效果。方法：以2016年1月-12月在我院确诊并接受治疗的62例CAP患者为研究对象，按照入院先后顺序分为参照组、研究组，各31例，两组患者入院后均予以常规治疗，研究组在常规治疗基础上予以纤支镜肺泡灌洗联合抗生素治疗，对比效果。结果：治疗前两组患者血氧分压（PaO2）、动脉血二氧化碳分压（PaCO2）、白细胞计数对比差异均无统计学意义，（P0.05）；治疗后研究组患者的PaO2水平明显高于参照组，PaCO2、白细胞计数水平明显低于参照组，研究组临床治疗总有效率为90.32%（28/31）明显高于参照组的67.74%（21/31），对比差异均有统计学意义，（P0.05）。结论：纤支镜肺泡灌洗联合抗生素治疗CAP，可有效改善患者的血气指标，降低白细胞计数，优化疗效，临床应用价值高。 　　【关键词】纤支镜肺泡灌洗；抗生素；社区获得性肺炎 　　Abstract Objective： To explore the clinical effect of bronchoalveolar lavage combined with antibiotic therapy （CAP）. Methods ： 62 patients with CAP in -12 months of January 2016 in our hospital were diagnosed and treated as the research object， according to the order of admission into the study group and control group， 31 cases each， two groups of patients were given conventional treatment after admission， the study group on the basis of routine treatment of fiberoptic bronchoscopy with bronchoalveolar lavage combined with antibiotic therapy. Contrast effect. Results：Two groups of patients before treatment， blood oxygen partial pressure （PaO2）， arterial partial pressure of carbon dioxide （PaCO2）， white blood cell count comparison differences were no statistical significance （P 0.05）； patients in the study group after treatment PaO2 levels were significantly higher than that of the control group， the level of white blood cell count PaCO2， significantly lower than the control group and study group the clinical total effective rate was 90.32% （28/31） was significantly higher than that of the control group 67.74% （21/31）， compared the differences were statistically significant （P 0.05）. Conclusion： bronchoscopic alveolar lavage combined with antibiotics in the treatment of CAP can effectively improve blood gas index， reduce white blood cell count and optimize curative effect， and has high clinical application value. 　　Key words： bronchoscopic alveolar lavage； antibiotics； community-acquired pneumonia 　　【中?D分类号
