重症患者血清瓜氨酸水平与胃肠损伤相关性研究 　　【摘要】 目的：探讨重症患者血清瓜氨酸水平与胃肠损伤的相关性。方法：以本院2014年6月-2015年6月入院的21例脓毒症患者作为研究组，记录患者的胃肠损伤，检测患者血瓜氨酸浓度、降钙素原、C-反应蛋白，选取20例健康体检者作为对照组。采用APACHEⅡ评分和SOFA评分来评估患者的疾病严重程度。结果：研究组依据AGI分级的各亚组患者的血清瓜氨酸浓度均明显低于对照组，而PCT及CRP水平均明显高于对照组，且患者的血清PCT、CRP水平和APACHEⅡ评分、SOFA评分均随着患者AGI等级升高而逐渐升高，各组患者以上观察指标比较差异均有统计学意义（P0.05）。血清瓜氨酸与AGI分级、APACHEⅡ评分、SOFA评分、CRP以及PCT呈显著负相关（P0.05）。结论：瓜氨酸是评估重症患者胃肠损伤、疾病严重程度以及预后的有效特异性指标。 　　【关键词】 重症患者； 血清瓜氨酸； 胃肠损伤 　　A Related Study between Serum Citrulline Levels and Gastrointestinal Injury in Severe Patients/WEI Hui-ping，WANG Yu，JIN Pei，et al.//Medical Innovation of China，2016，13（09）：070-074 　　【Abstract】 Objective：To investigate the correlation between serum citrulline levels and gastrointestinal injury in severe patients.Method：Twenty-one sepsis patients in our hospital from June 2014 to June 2015 were selected as the research group，and recorded these patients’ gastrointestinal injury，the serum concentration of citrulline，calcitonin fibrinogen，C-reactive protein of patients were detected，and twenty healthy persons were selected as the control group，the severity of disease in patients were evaluated by APACHEⅡ score and SOFA score.Result：In accordance with AGI grade，the serum citrulline levels of each subgroup in the research group were significantly lower than the control group，the PCT and CRP levels of each subgroup were significantly higher than the control group，and the patients’ serum PCT，CRP level and APACHEⅡ score，SOFA score increased with the patients’ AGI level and gradually increased，there were significant differences in the observed indicators between the groups（P0.05）.The citrulline and AGI grade，APACHEⅡ score，SOFA score，CRP and PCT showed significant negative correlation（P0.05）.Conclusion：Citrulline is an effective specificity index to evaluate the severity and prognosis of gastrointestinal injury in severe patients. 　　【Key words】 Severe patients； Serum citrulline； Gastrointestinal injury 　　First-author’s address：Affiliated Hospital of Gansu
