急性冠状动脉综合征合并严重心律失常112例疗效与安全性分析 　　【摘要】 目的 评价胺碘酮治疗ACS合并快速型心房纤颤、心房扑动或室性心律失常的疗效及安全性。方法 对112例ACS合并快速型心房纤颤、心房扑动或室性心律失常的患者，先用胺碘酮150 mg静脉注射后，必要时追加150 mg，续以1．0 mg/min滴注，维持6 h,后以0．5 mg/min速度维持、总量为1 050~1 500 mg。以后口服胺碘酮，第1周600 mg/d，第2周400 mg/d，第3周200 mg/d维持。疗程共4周。结果 100例患者有效，起效时间为0．5~24 h。结论 静脉加口服胺碘酮治疗ACS合并快速型心房纤颤、心房扑动或室性心律失常安全有效。? 　　【关键词】胺碘酮；急性冠状动脉综合征；心律失常；分析 　　 　　Study of effect and security of amiodarone on treating severe arrhythmia with acute coronary syndrome 　　 　　SU Jing-qiang.Department of Cardiology,The Panyu District People’s Hospital,Guangzhou 511400,China 　　? 　　【Abstract】 Objective To evaluate the efficiency and the safety of amiodarone for the treatment of atrial fibrillation with rapidventricular response,atrial flutter with rapid ventricular response,or ventricular arrhythmia in patients with acute coronary syndrome(ACS).Methods 112 patients with the diseases mentioned above treated in our hospital were selected as the study objects.The first bolus of 150 mg amiodarone was given intravenously,repeated with the same dose if no efficacy shown.Later intravenous drip was given by 1．0 mg/min in 6 h,and then by 0．5/min.The total dosage was 1050-1500 mg.Then oral amiodarone was given 600 mg/d the first week,400 mg/d the second,200 mg/d the third and the forth.Results 100 cases were effective in 0．5~24 h.Conclusion Intravenous and oral amiodarone is secure and effective in the treatment of atrial fibrillation with rapid ventricular response,atrial flutter with rapid ventricular response,or ventricular arrhythmia in patients with acute coronary syndrome.? 　　【Key words】Amiodarone;Acute Coronary Syndrome;Arrhythmia;Analysis 　　急性冠状动脉综合征(ACS)合并严重心律失常时，易导致死亡，是心内科需要紧急处理的常见病之一。自2005年1月至2008年1月应用胺碘酮治疗ACS合并严重心律失常，发现其具有较好的疗效和安全性，现报告如下。? 　　1 资料与方法? 　　1．1 一般资料 2005年1月至2008年1月，本院心内科收治ACS合并严重心律失常112例，年龄38~80(59±21)岁，男52例，女60例。其中NYHA心功能Ⅰ级10例，心功能Ⅱ级55例，心功能Ⅲ级32例，心能Ⅳ级15例。类型:快速型心房纤颤31例，快速心房扑动19例，频发室性早搏、多源室性早搏、连续室性早搏45例，阵发性室性心动过速17例。不稳定型心绞痛30例，急性非ST段抬高型心肌梗死20例，急性ST段抬高型心肌梗死62例。对合并
