CCP抗体、AKA及APF对类风湿性关节炎的诊断价值 　　【摘要】 目的：研究CCP抗体、AKA、APF对类风湿性关节炎（RA）的诊断价值。方法：选取笔者进修单位云南省第一人民医院2016年3-9月收治的疑似类风湿性关节炎患者200例为本次研究对象，对受检对象进行CCP抗体、AKA及APF检测。结果：CCP抗体诊断准确率为93.00%，显著高于AKA的79.50%和APF的76.00%，比较差异均有统计学意义（P0.05）；两项指标联合和三项指标联合诊断的灵敏度、特异度比较差异无统计学意义（P0.05），而三项指标四项联合方式的灵敏度均低于CCP抗体的灵敏度。结论：在类风湿性关节炎的诊断中，CCP抗体的诊断价值最高，而三项指标的联合检测并不能明显提高诊断价值，临床上应根据患者的临床症状、体征、影像学结果、验室检查等综合确诊，减少漏诊、误诊病例。 　　【关键词】 CCP抗体； AKA； APF； 类风湿性关节炎 　　doi：10.14033/j.cnki.cfmr.2018.3.032 文献标识码 B 文章编号 1674-6805（2018）03-0062-02 　　Diagnostic Value of Anti-CCP，AKA and APF on Rheumatoid Arthritis/YAN Jixiao.//Chinese and Foreign Medical Research，2018，16（3）：62-63 　　【Abstract】 Objective：To study the diagnostic value of anti-CCP，AKA and APF on rheumatoid arthritis.Method：From March to September 2016，200 patients with suspected rheumatoid arthritis were selected from The First People’s Hospital of Yunnan Province as the object，anti-CCP，AKA and APF were detected in the subjects.Result：The diagnostic accuracy of Anti-CCP was 93.00%，which was significantly higher than 79.50% of AKA and 76.00% of APF，the differences were statistically significant（P0.05）.There was no significant difference in the sensitivity and specificity of the combined diagnosis of the two indicators and the three indicators（P0.05），while the sensitivity of the four combinations of the three indicators was lower than the sensitivity of the CCP antibody.Conclusion：In the diagnosis of rheumatoid arthritis，anti-CCP as the highest diagnostic value，but the combined detection of three indexes does not significantly improve the diagnostic value，and should according to clinical symptoms，signs，imaging findings，laboratory tests in patients with comprehensive diagnosis，reduce misdiagnosis cases. 　　【Key words】 Anti-CCP； AKA； APF； Rheumatoid arthritis 　　First-author’s address：Qujing Traditional Chinese Medicine Hospital，Qujing 655000，China 　　类风湿性关节炎（RA）是指以关节损伤为主的慢性免疫系统疾病，其致残率较高，而早期诊断和早期治疗有助于改善预后，预防残疾的发生[1-2]。近些年来，不少学者的研究指出RA患者血清中存在多种抗体，如：抗角蛋白抗体（AKA）、抗环瓜氨酸肽（
