PAGE PAGE 1 摘 要 非小细胞肺癌组织中 ERCC1 表达与铂类化疗疗效 及预后分析 研究生：曾薇 导师：单利 副教授 摘 要 目的：研究探讨非小细胞肺癌（non-small cell lung cancer，NSCLC）组织中 DNA 切除修复交叉互补基因 1（excision repair cross-complementing 1，ERCC1）的表达与 铂类化疗远期疗效之间的关系，并观察该指标及相关临床因素对患者预后的影响。 方法：应用免疫组织化学方法对 2003 年 1 月～2006 年 12 月我院 63 例 NSCLC 手术 切除石蜡包埋病理标本中 ERCC1 基因表达 进行了测定。组间样本率的比较采用 Pearson x2 检验或连续性效正 x2 检验，采用 Kaplan-Meier 法进行生存率的计算，组 间生存曲线的比较采 log-rank 检验，影响 NSCLC 患者预后的相关因素分析采用 Cox 多因素回归分析法。以上数据采用 SPSS13.0 统计软件处理。结果：NSCLC 组织中 ERCC1 阳性表达率为 26.9%；ERCC1 表达与患者的性别、年龄、p- TNM 分期及病理 类型无关；ERCC1 表达阴性者术后使用铂类药物辅助化疗有延长其生存时间的趋势； ERCC1 表达阳性者术后使用铂类药物辅助化疗不能延长其生存时间；p-TNM 分期、 ERCC1 表达为影响 NSCLC 患者术后生存期的独立因素。结论：ERCC1 表达阴性者 术后使用铂类药物辅助化疗可能获得生存受益；ERCC1 可成为判断 NSCLC 预后的 一项重要指标。 关键词：非小细胞肺癌； 切除修复交叉互补基因 1； 化疗； 预后 新疆医科大学医学硕士学位论文 Expression of ERCC1 and Platinum-based Chemotherapy and Prognosis of Non-small Cell Lung Cancer Postgraduate: Zeng Wei Supervisor: vice professor Shan Li Abstract Objective: To study the relation between expression of excision repair cross- complementing gene 1 (ERCC1) in tissue of non-small cell lung cancer and long-term effect of platinum-based chemotherapy. And to observed the expression of ERCC1 and other related clinical factors impacted on prognosis of NSCLC. Methods: Expression of ERCC1 in 63 cases of NSCLC paraffin-embedded pathological specimens was determined by Immunohistochemistry method in January 2003 to December 2006 in our hospital. SPSS 13.0 for windows software was used for statistics, x2 test was used in the comparisons of rate, data were examined by Kaplan-Meier method and log-rank test. The Cox regression model was used to evaluate the expression of ERCC1 and other related clinical factors influenced in survival rate. Results: Positive rate of ERCC1 in NSCLC was 26.9% and it did not related with gender, age, p-TNM stage and pathological type.
