胎膜早破中阴道唾液酸酶和血清ＩＣＡＭ－１水平联检临床意义 　　【摘要】 目的 探讨阴道唾液酸酶和血清细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)水平联检在胎膜早破中的临床意义及两者之间的关系。方法采用比色法对阴道分泌物进行阴道唾液酸酶检测和酶联免疫方法检测158例足月前胎膜早破、96例足月胎膜早破孕妇血清ICAM-1的表达情况，选取正常足月孕妇80例为对照组，观察结果并进行统计学处理。结果 胎膜早破组孕妇阴道唾液酸酶检测阳性率及血清中ICAM-1 水平均明显高于正常足月孕妇组，统计学分析显示两组间差异有统计学意义(P0．05)。结论 联合阴道唾液酸酶和血清ICAM-1检测在临床预测胎膜早破有一定的应用价值。 　　【关键词】 胎膜早破；阴道唾液酸酶；细胞间黏附分子-1(ICAM-1) 　　 　　The clinical significance of ICAM-1 in serum and vaginal sialidase measured in premature rupture of membrane 　　 　　YANG Zhi-hui,LI Quan-xiang.Maternal and child Care Hospital in Baoding,Hebei 071000,China 　　 　　【Abstract】 Objective To investigate the expression and clinical significance of intercellular adhesion molecule-1(ICAM-1)in serum and vaginal sialidase measured in premature rupture of membrane.Methods ICAM-1 in serum was determined by enzyme-linked immunosorbent assay(ELISA)and vaginal sialidase was measured by colorimetry in 158 cases of preterm premature rupture of membrane,96 cases of Premature rupture of membrane and 80 cases of normal term gravida.The relativity ICAM-1 in serum and vaginal sialidase measured were analyzed.Results The expression of ICAM-1 in serum and Vaginal sialidase masccline percentage in premature rupture of membrane was higher than that in normal gravida.There were significant differences in the two groups(P0．05).Conclusion Detection of the ICAM-1 in serum and vaginal sialidase measured maybe play important roles in premature rupture of membrane. 　　【Key words】 　　Premature rupture of membrane;Vaginal sialidase;Intercellular adhesion molecule-1(ICAM-1) 　　 　　胎膜早破是产科常见的并发症，是难免早产以及围产儿患病及死亡的主要原因之一，而胎膜早破主要是由于感染造成。因此找到一种对胎膜早破早产有预测价值的指标非常重要。有研究表明，细胞间黏附分子-1(ICAM-1)是介导白细胞与内皮细胞黏附的主要分子之一。广泛表达在各种血源性和非血源性细胞的表面，可以受各种因素的影响而呈诱导性表达。本研究旨在探讨胎膜早破孕妇阴道唾液酸酶检测以及血清ICAM-1 的含量变化及两者之间的关系并为临床治疗提供理论依据。 　　 　　1 资料与方法 　　 　　1．1 研究对象 选择本院2006年6月至2007年12月住院分娩的158例足月前胎膜早破、96例足月胎膜早破患者，年龄23~29岁，平均26．8岁，取同期住院分娩的正常足月孕妇80例为对照组。3组孕妇均为单胎、头位、 初产妇,无内外科并发症。 　　1．2 测定方法 常规消毒外阴，窥阴器扩张阴道，用无菌棉拭子从阴
