重型颅脑损伤合并静脉窦损伤临床特点及紧急处理策略 　　【摘要】 目的 探讨颅脑损伤合并静脉窦损伤患者的临床特点及紧急处理策略。方法 回顾性分析2004年1月至2007年7月我院收治的26例重型颅脑损伤合并静脉窦损伤患者的临床特点及紧急处理方法。结果 所有患者均在术中完成静脉窦损伤的紧急修补，止血明确。26例中恢复良好15例、中残7例、重残1例、死亡3例。结论 根据手术中静脉窦具体的损伤特点采用相应的修补方法，尽早控制致命性大出血，恢复静脉窦血流，可显著改善颅脑损伤合并静脉窦损伤患者的预后。 　　【关键词】 颅脑损伤；颅内静脉窦；手术治疗；预后 　　 　　The clinical feature and surgical strategies for head injuried patients with dural sinus injury 　　 　　LIN He-lin,TANG Kai,LIN Xiao-feng.Department of Neurosurgery,the Xixiang People’s Hospital of Shenzhen，Guangdong 518102，China 　　 　　【Abstract】 Objective To study the clinical feature and surgical techniques in head injuried patients with dural sinus injury.Methods 26 head injuried patients with dural sinus injury who admitted to our department from Jan.2004 to July 2007 were analyzed retrospectively.Results 26 cases(88．46%)survived in which 15 cases had a good recovery，7 cases had a moderate disability,and other 1 case with severe deficits.The other 3 dead after therapy.all of them received prompt treatment to repair the injuried sinus during the surgery.Conclusion The prognosis ofhead injuried patients with dural sinus injury could be improved by taking approaches to repair the sinus by the types of lesion that can be identified during the surgery,stopping the lethal bleeding and restoring the dural sinus blood flow. 　　【Key words】 Head injury;Dural sinus;Surgery;Prognosis 　　 　　自2004年1月至2007年7月，我院进行颅脑损伤开颅手术502例，其中26例为颅脑损伤合并静脉窦损伤，这些患者多在术中才发现静脉窦破裂出血，而术前影像学特点并不典型。现就其临床特点，术前影像学征象及术中处理方法报告如下。 　　 　　1 临床资料 　　 　　1．1 一般资料 本组26例中，男17例，女9例;年龄16~60岁，平均30岁。受伤原因:车祸伤13例，坠落伤8例，其他5例。受伤机制:皆为直接损伤，其中减速伤11例，加速伤15例。着位部位:顶部12例，枕部6例，颞部8例。 　　1．2 临床表现 术前皆有不同程度的意识变化，GCS8 分12例，6~8分8例，3~5分6例。术前单侧瞳孔散大11例，双侧瞳孔散大3例，无瞳孔散大12例。 　　1．3 影像学检查 头颅CT显示:双额底硬膜下血肿合并脑挫裂伤3例，单侧硬膜下血肿14例，单侧硬膜下血肿并对侧脑内血肿4例，单纯脑肿胀3例。颞底部硬膜外血肿2例，其中5例并有前纵裂增宽积血。14例患者入院时较危重，术前未行头颅X线片检查。 　　1．4 手术部位 额部冠状开颅4例，单侧额颞顶部开颅18例，颞部开颅4例。其中硬膜下血肿清除21例，硬膜外血肿清除2例，单纯去大骨瓣减压术3例。 　　1．5 术中情况 术中发现上矢状窦损伤13例，窦汇损伤3例，横窦损伤8例，岩上窦损伤2例。出现脑膨出6例，低血压5例，出血量约在100~2 000 ml。 　　1．6 治
