肿瘤代谢与营养电子杂志  2015年9月第2卷第3期              Electron J Metab Nutr Cancer，Sep. 2015，Vol. 2， No. 3 ·73· ·综述· 肝癌患者营养不良的原因及其营养治疗 易佳盛，张吉翔，王静，董卫国（武汉大学人民医院消化内科，武汉 430060） 摘要：目的 肝癌患者容易出现营养不良，严重营养不良增加了肝癌治疗相关不良反应发生率和死亡率，降低了肝癌治 疗的效果和患者的生活质量，延长了住院时间。本文介绍了肝癌引起营养不良的原因以及营养治疗方法，旨在为临床上肝 癌营养治疗提供指导。方法 以肝癌、营养不良、营养治疗在CNKI 和PubMed 中查阅相关文献，对肝癌引起营养不良原因 和营养治疗方法进行归纳总结。结果 肝细胞受损、食欲减退、物质代谢受影响及放疗和化疗等影响机体消化吸收，是引起 肝癌患者营养不良的主要原因。肝癌患者的疾病状态和治疗方式不同，各种营养治疗方法效果也差异明显。结论 肝癌患者 出现营养不良的原因涉及多个方面，肝癌营养治疗方案要根据患者状况，合理、科学的选择。 关键词：肝癌；营养不良；营养治疗 Malnutrition and nutritional therapy of hepatocellular carcinoma YI Jia-sheng, ZHANG Ji-xiang, Wang Jing, DONG Wei-guo Dept.of Gastroenterology, Renmin Hospital of Wuhan University, Wuhan, 430060, China Abstract: Objective Malnutrition commonly appears in patients of hepatocellular carcinoma. Severe malnutrition contributes to raising the risk of therapy-associated side effects and mortality, decreasing the efficacy of anticancer treatments and increasing the length of hospital stay. This paper introduced the causes of hepatocellular carcinoma-related malnutrition and the nutritional therapies of hepatocellular carcinoma to provide guide for its use in clinic. Methods Using hepatocellular carcinoma, malnutrition and nutritional therapy to search the literatures in the CNKI and PubMed. Then summarizing the causes of hepatocellular carcinoma- related malnutrition and the nutritional therapies. Results The damage of liver cell, anorexia, metabolism and assimilation are the main causes of hepatocellular carcinoma-related malnutrition. And the therapeutic effect varies with the clinical status and cancer therapies. Conclusions The causes for hepatocellular carcinoma-related malnutrition are multifarious and the choices of scientific and efficient nutritional therapies should be in accordance with the patient status.
