摘要 摘要 万方数据 万方数据 摘 要 目的：甲状腺疾病是临床上常见的疾病，促甲状腺激素（TSH）是诊断甲状 腺疾病的关键，目前临床上使用的 TSH 值参考范围建立来源于无筛选人群，因 此 TSH 参考范围备受争议。本研究目的为重新建立江西地区健康成年人 TSH 参 考范围，并分析影响 TSH 参考值范围的因素。 方法：收集 2012 年 7 月-2013 年 9 月来自江西省 11 个地级市共计 1000 名 健康、未孕志愿者参加此次研究，进行了问卷调查，并分别测试了 TSH、甲状腺 自身抗体[抗甲状腺过氧化物酶抗体（TPOAb）和甲状腺球蛋白抗体（TgAb）]， 进行了甲状腺触诊及超声体检。采用 SPSS17.0 软件，统计正常参考范围测定采 用 95%参考区间计算，用 Kolmogorov-Smirnov 进行正态性检验，各组间 TSH 差 异比较采用 t 检验，评估性别、甲状腺自身抗体及其他病史等因素对 TSH 值参 考范围的影响，年龄组之间及 BMI 组间两两比较采用 LSD 检验。 结果：排除了小于 18 岁、TPOAb 阳性、甲状腺肿大、近期服用含碘食物、 甲状腺疾病史以及家族史等因素后获得 463 名志愿者。结果为：在正常人群中 TSH 值非正态分布，向右偏斜，取对数后呈正态分布。在这个研究中年龄大于 18 岁健康的 463 名志愿者建立的新的参考范围是 0.59-4.95 mIU/L。女性血清 TSH 参考范围 0.68-5.21mIU/L，男性 0.55-4.76mIU/L，女性明显高于男性，差 异有统计学意义（P0.05）。甲状腺抗体对 TSH 参考范围有影响（P0.05）；而 年龄、甲状腺肿大、BMI 及近期服用含碘食物对 TSH 参考范围没有影响（P0.05）。 结论：1、性别、甲状腺抗体会影响 TSH 参考范围的建立。 2、本研究的统计结果显示 TSH 上限值及下限均升高，不同于厂商提 供的参考范围，各个地区及实验室均应建立本地区 TSH 参考范围。 关键词：TSH；参考范围；成年人；甲状腺抗体 I Ab Abstract Abstract Objective：Thyroid disease is a common clinical disease and TSH is the key to the diagnosis of thyroid diseases. But the current serum TSH reference intervals is controversy as they were established from unselected background populations. The purpose of the subject is to redefine the TSH reference interval in the healthy adults in Jiangxi Province and analyze the influence factors of TSH reference value range. Methods: Serum TSH concentrations were derived from 1000 healthy, non-pregnant Jiangxi people. Firstly, the questionnaire survey was conducted.
