·论著· CRBP病情评估法急诊临床应用研究1 王连馥 闫波 姜正伟 【摘要】 目的 选择意识状态（consciousness）、呼吸(respiratory)、血压(blood pressure)和脉搏(pulse)4项生命体征作为病情严重程度及病情变化趋势的评估方法（即CRBP病情评估法，简称CRBP方法）与MEWS（改良的早期预警评分）评分法（以下简称MEWS评分法）在急诊急危重病患者的病情评估的效能进行比较。方法 选取2009年9月1日至2011年1月28日，我院急诊科的急危重症抢救病例1819例作为研究对象，对本组病例进行意识状态、呼吸、血压、脉搏等生理指标监测，采用CRBP病情评估法评估病情、指导诊断治疗，并对意识状态、收缩压、呼吸、体温及心率等生理指标监测，用MEWS评分法评分，并与CRBP方法对照研究，研究其相关性。结果 CRBP方法潜在危险病人和普通病人与MEWS评分法中评分5分的患者无显著性差异；CRBP方法中急危重患者与MEWS评分法中评分9分的患者无显著性差异，CRBP方法与MEWS评分法之间的相关系数：0.717，P0.001，有统计学意义，表明CRBP方法与MEWS评分法呈正相关。结论 CRBP方法不需要评分，简单快速，更适合急诊分诊、急救批量伤病分拣分类，早期识别潜在危重症，有利于急诊科的病人管理和医疗资源的合理利用；也适用于急诊急救，能反映疾病严重程度及病情变化方向；也适用于急危重症患者的预后、转归预测，客观评价危重患者可能发生的危险性概率。 【关键词】 病情评估；CRBP方法；MEWS评分法；急诊抢救；急诊分诊 The clinical application study of CRBP evaluation method WANG Lian-fu, YAN Bo,JIANGg Zheng-wei. Aerospace central Hospital, Beijing 100049，China 【Abstract】 Objective Compare the effect of 4 vital signs evaluation method（refer to conscious state, respiratory, blood pressure and pulse，CRBP）and MEWS (modified early warning score) evaluation method in emergency and critical illness. Methods Chose 1819 cases of emergency and critical illness patients from September 1, 2009 to January 28, 2011 in our emergency department, to monitor conscious state and respiratory and blood pressure and pulse, and so on. Applying CRBP evaluation methods to evaluate the condition and instruct diagnosis and treatment. And monitor conscious state, systolic pressure, respiratory, temperature and heart rate, applying MEWS evaluation method to score and study the correlations of both methods. Results There were no significant differences between the potential dangerous and ordinary patients applying CRBP evaluation methods and the patients score5 points applying MEWS evaluation method. There were no significant differences between the very dangerous patients applying CRBP evaluation methods and the patients score9 points applying MEWS evaluation method. The correlation coefficient between CRBP evaluation method
