阻塞性睡眠呼吸暂停低通气综合征18例临床分析 (贵阳市南明区人民医院老干综合科 贵州 贵阳550002) 【摘 要】目的：通过检测阻塞性睡眠呼吸暂停低通气综合征(osAHs)患者 血清血管内皮生长因子(VEGF)质量浓度，探讨其与肺动脉高压的关系。方法：选 择2010年4月?2014年8月我院收治的18例因打鼾就诊的患者，次日晨7： 00 测定其血清VEGF质量浓度,并予超声心动图检测OSAHs患者肺动脉压力。结果： PsG检测结果显示与单纯鼾症组比，OsAHs组存在夜间低氧；且重度组比轻中度 OSAHS组夜间低氧更严重(均有统计学意义)。与单纯鼾症组比较，VEGF质量浓度 在重度OsAHs组中明显升高(Plt;O? 05),并分别与衡量病情及夜间低氧情况指 标呈正相关，与夜间最低经皮血氧饱和度(SpO：)呈负相关。结论：中重度OSAHS 患者存在夜间低氧饱和度，同时有VEGF增高。 【关键词】睡眠呼吸暂停综合征；血管内皮生长因子；肺动脉高压 【中图分类号】R766 【文献标识码】B 【文章编号】1003-5028 (2015) 6-0639-01 [Abstract] Objective： through the detection of obstructive sleep apnea hypopnea syndrom (osAHs) patients with serum vascular endothelial growth factor (VEGF) mass con centratio n, explore its relati on ship with pulm on ary hypertensi on. Methods： in April 2010 - August 2014 18 cases of our hospital for patients with snoring visits, the next morning at 7:00 measure its quality of serum VEGF con centrati on, pulm on ary artery pressure and echocardiography to detect 0 sahs patients. Results： PsG results show than to the simple sleep apnea group, 0 sahs group of low oxygen at night; And severe hypoxia group than in mild-to?moderate 0 sahs night more serious (have statistical significanee). Compared with pure sleep apnea group, VEGF concen tration in severe 0 sahs group in creased sign ificantly (P lt; 0.05), and respectively with measure and night low oxygen index were positively correlated, and at night the lowest percutaneous oxygen saturation (SpO) showed a negative correlation. Conclusions： relation between 0 sahs patients exist low oxygen saturati on at n ight, at the same time have in creased VEGF. 【Key words] sleep apnea syn drome; Vascular en dothelial growth factor; Pulm on ary arterial hypertension 阻塞性睡?眠呼吸暂停低通气综合征(OsAHs)是一种发生率极高的疾病，具 有一定潜在的危险性。OsAHs患者由于有夜间频繁的呼吸暂停、低通气发生，存 在不同程度的夜间低氧血症，甚至有白天低氧的发生。血管内皮生长因子(VEGF) 属生长因子中血小板衍化生长因子家族，它能特异性地作用于血管内皮细胞，具 有促进内皮细胞增殖分化、新生血管形成及增加微血管通透性等功能，而低氧是 VEGF表达的强烈诱导因素。目前OSAHs和心血管疾病密切关系日益受到人们的 关注,OsAHs和VEGF及OsAHs和肺动脉高压的关系已有了一些报道,但有关VEGF 与OsAHs患者肺动脉高压的关系国内外均未见
