胫骨交锁髓内钉结合阻挡螺钉技术在胫骨近端骨折中的应用 徐志斌 王进 （武威市人民医院骨一科，甘肃 武威 733000） 摘要 目的 观察交锁髓内钉结合阻挡螺钉技术治疗胫骨近端骨折的临床疗效。方法 自2010年1月至2012年12月，12例胫骨近端骨折患者，男性8例，女性4例，年龄17～56岁，平均34.8岁。左侧5例，右侧7例，致伤原因：交通事故9例，高处坠落伤1例，直接打击伤1例，均采用闭合复位交锁髓内钉结合阻挡螺钉技术治疗，术后评估其临床疗效。结果 12例患者均获得32～60周随访，平均随访时间46.8周，所有患者伤口均一期愈合，术后未出现伤口感染，术后第4周及12周时复查患肢X线片，见骨折均对位对线良好，固定稳定。骨折均获得愈合。结论 采用阻挡螺钉技术在胫骨交锁髓内钉治疗胫骨近端骨折中提高了骨折复位质量，增加了骨折断端稳定性，临床效果满意。 关键词 胫骨近端；阻挡螺钉；内固定 Application of Interlocking Intramedullary Nail Combined with Blocking Screw in Proximal Tibial Fractures XU zhi-bin，WANG jin. The People’s Hospital of Wuwei City, wuwei 733000,Gansu，China ABSTRACT Objective: to observe the clinical effects of interlocking intramedullary nail combined with blocking screw in proximal tibial fractures. Method: 12 patients with proximal tibial fractures treated from January 2010 to December 2012 were selected as the research objects, including 8 male patients and 4 female ones. Their ages were between 17 and 56, with the average of 34.8 years old. Their fractures were in the left side in 5 cases and in the right side in 7 cases. As for the injury causes: there were 9 cases caused by the traffic accident, 1 case by falling from high place, and 1 case by direct strike. They were all treated with interlocking intramedullary nail combined with blocking screw and their clinical effects were evaluated after the operation. Results: the 12 patients all received follow-up visit for 32-60 weeks, with the average of 46.8 weeks. The wound of all patients were all healed up in the first phrase and wound infection did not occur after operation. In the re-examination with X-ray in the fourth and twelfth weeks, it could be seen that the counterpoint and alignment of the fractured bones were all very good and they were fixed and steady. The fractures all got union and healing.
