浅析门诊西药房不合理用药处方分析及改进措施 湖南省常德市安乡县人民医院药剂科门诊丙药房湖南安乡415600 【摘要】目的：分析门诊丙药房用药处方的不合理之处，追宄原因并提出相应 的改进措施，规范门诊处方管理。方法：整理分析我院2013年1月至2015年 12月的门诊处方17234张，检查门诊处方的用药规范现状，按照处方的质量将 其划分为良好，不适宜，不合格三个级别。分析处方不合理的具体表现及其原因， 讨论后提出改进措施，以期规范门诊处方管理，提高处方的准确性、合理性及适 用性。结果：检查的处方中，质量良好的处方占比98.22%，不合规的处方占比 1.34%,主要的不合理表现为：处方上临床诊断书写不全或未书写、药品剂型或 药型不适宜、用药时间不适宜、剂量不适宜、联合用药不当等；不适宜的处方占 比0.38%,主要表现为：无合理理由开具药理相同的两种药物、无适应症给药。 结论：门诊丙药房用药处方存在较大问题，关系到患者的临床治疗效果及经济财 产问题，应当对出现的不合理现象进行分析讨论，实行整改措施，保证门诊处方 用药合理性，保证患者的人身及财产安全。贯彻医德，营造和谐的医患关系。 【关键词】门诊用药处方；不合理；改进 Analysis and improvement measures of irrational drug use in outpatient pharmacy Abstract： Objective： to analyze the unreasonable prescriptions in outpatient pharmacy, investigate the reasons and put forward the corresponding measures to improve the management of outpatient prescriptions.Methods： 17234 outpatient prescriptions in our hospital from January 2013 to December 2015 were analyzed, and the current situation of drug use in outpatient prescriptions was checked.The quality of the prescriptions was divided into good, unsuitable and unqualified three grades.The specific manifestations and reasons of the irrational prescription are analyzed, and the improvement measures are put forward to standardize the management of outpatient prescriptions in order to improve the accuracy, rationality and applicability of the prescription.Results： check the prescription and prescription of good quality accounted for than 98.22%，sub accounting for prescription of compliance than 1.34%, the main unreasonable showed： prescription writing of clinical diagnosis not or not writing, pharmaceutical dosage forms and drug is not suitable, medication time is not suitable, unsuitable dose, combined medicine improperly etc.； inappropriate prescriptions accounted for than 0.38%, mainly for： no reasonable grounds for the issuance of the pharmacological the same of the two drugs，no indications to medicine.Conclusion： outpatient pharmacy prescription medication, there are big problems, re
