屮国病案 2017,11(18),76-80 基于病例分型的子宫下段剖宫产住院费 用分析 沈鑫李虹张凌燕孙杨 陕西省人民医院 导出/参考文献关注 分享 收藏打印 摘要： 目的比较不同病例分型子宫下段剖宫产患者住院费用差异，分析子宫下段剖宫 产住院费用影响因素，为医疗费用控制提供参考。方法选取某三甲医院2015 年6月1 R-2016年5月31 FI共2820例子宫下段剖宫产住院患者，应用计算机 初筛，结合专家病案复判，确定分型后方差分析、结构变动度比较各组费用和结 构差异，最后多元线性回归分析住院费用影响因素。结果病例分型后A型、B 型、C型、D型分别为1155例、920例、392例、353例。方差分析提示不同组 间住院费用间有差异(F二106.392)。结构变动度分析提示各组间费用结构变动 从大到小依次为 A-D (33. 06) , B-D (30. 34) , A-C (18. 83) , B-C (18. 38), C-D (17.99) , A-B (3.68)。多元回归分析提示影响住院费用因素为入院情况、 住院天数、并发症和合并症情况、病例分型。结论 基于病例分型对病种费用的 判定科学、便捷、合理，可为医疗政策改革提供参考。 关键词： 病例分型;剖宫产;住院费用; 收稿日期：2017-05-26 基金：陕西省人民医院院内孵化基金(2015025) Analysis on the Hosp it aliza tion Cos t of Cesarean Section in Lower Uterine Segment Based on Case Classification Shen Xin Li Hong Zhang Lingyan Sun Yang Shanxi Provincial Peoples Hospital; Abstract： Objectives To compare the differences in hospitalization costs of cesarean section in lower uterine segment in different case classification types, analyze the factors that affecting the cost of cesarean del ivery in the 1 ower uterine segment, providing a reference for medical cost control Methods To collcct 2820 cases of utcrinc cesarean section hospitalized patients from 1 st June 2015 to 31 st May 2016 in a Three A and Tertiary Hospital, apply computer screening combined with expert using medical records to determine the types. Using varianee analysis and degree structure variation analysis of the cost and the structure in different groups, and make multiple linear regression analysis of hospitalization costs finally. Results There were 1155 cases, 920 cases, 392 cases and 353 cases of type A (simple type) , type B (type of acute obstruction) , type C (complicated and difficult type) and type D (complex critical type) respective!y.
