Health and Safety RequirementsLafarge Iligan Inc.;Annex B;Annex B;Presentation name or chapter;The CONTRACTORS’ lifting equipment (gears, slings, shackle, crane, forklift, manlift, boom truck) and heavy equipment (payloader, bobcat, bulldozer, backhoe, etc) shall have 3rd party certification from COMPANY accredited certifying body (TESDA, ICIP, 1st Philippines, Wire Rope, etc.) The CONTRACTOR shall assign at least One (1) Registered Safety Officer for every 50 workers on site, however, if the contractor is doing 2 high risk jobs at the same time , one (1) Registered Safety Officer is required on each job. The CONTRACTOR must conduct Risk Assessment of all works and provides details of the environment, occupational health (includes drug testing program) and safety implications of the work to be undertaken, including all relevant MSDS. CONTRACTORs personnel who may be exposed to particular hazards by COMPANY activities, shall be provided by the CONTRACTOR of adequate instruction about the hazards (and if necessary, training must be provided) prior to undertake such work. CONTRACTOR must ensure that all of its personnel : (1)possessed an up to date clearances (NBI, Barangay, Police); (2) covered with sss and employees compensation and liability insurance; and (3) has valid medical certificate (blood test, chest x-ray, ECG 35 yo) with one (1) year validity from a reputable COMPANY accredited hospital and or clinic. CONTRACTORs Site Safety Coordinator is required to attend safety meeting every week and submit a weekly safety listing at security. ;SUBCONTRACTING The CONTRACTOR must obtain approval from the COMPANY, prior to engaging a sub-Contractor and the CONTRACTOR must impose upon the sub-contractor the COMPANY’s safety standards, rules and regulations for CONTRACTOR. The CONTRACTOR must ensure that its SUB-CONTRACTOR possess all the Health and Safety Registrations, Certifications, Licenses, Training, and are adequately apprised by the CO
