The inflammation involves medullary portion of bone such as bone marrow, blood vessels and nerves, as well as haversian system, cortical bone and periosteum. In the orofacial region of the body, the most common location for this disease is the mandible. The maxilla, by virtue of its thin cortical plate and extensive blood supply, is less frequently compromised by osteomyelitis. Compromise of the blood supply is a critical factor in the establishment of osteomyelitis.? The mandible receives its major blood supply from the inferior alveolar artery. According to its etiology and clinical features, osteomyelitis of the jaws may be classified into three subtypes, including: (1) Pyogenic osteomyelitis(化脓性); (2) Special osteomyelitis (特异性); (3) Osteomyelitis from infection of physical or chemical necrosis of the jaws (物理化学因素造成的骨坏死继发感染的骨髓炎). Pyogenic osteomyelitis presents 90% cases in total of this disease and young people of 16~30 years old is the most involved population; Classification: (2) Marginal osteomyelitis(边缘性骨髓炎) Typically the inflammation begins with an space infection which erodes the outer surface of mandible body or ramus and thus inflammation extends to periosteum, cortical bone or even bone marrow sequently.
