Methods 80 patients with acute myocardial infarction treated in our hospital from January to December 2015 were selected and randomly and equally divided into r-PA group and UK group. Patients in r-PA group were treated by reteplase， and patients in UK group were given urokinase. The rate of thrombolysis coronary recanalization at 30 min， 60 min， and 120 min after treatment were recorded， the incidence rates of advee reaction and mortality rates in acute stage were compared between two groups， and the cardiac function and myocardial injury degrees were compared and analyzed. Results The recanalization rates of thrombolysis coronary at 30 min， 60 min， and 120 min after treatment were %， %， and % in the r-PA group， which were all significantly higher than those in the UK group（P）. The overall advee reaction rate in the r-PA group was %， the mortality rate was %， LVEF was （±）%， peak value of CTnI was （±） ng/mL， and time to peak value of CK-MB was（±） h， which were all significantly better than those in the UK group（P）. Conclusion Application of reteplase in treatment of acute myocardial infarction can significantly improve the cardiac function， improve recanalization rate of thrombolysis coronary， and reduce incidence of advee reaction。 [Key words] Re
