医院静配中心不合理医嘱点评及管理标准探讨
[摘要]目的分析该院静配中心不合理医嘱点评及管理标准。方法选择2017年1月—2018年12月该院静配中心的共52例抗肿瘤药物不合理医嘱为该次研究对象，对不合理医嘱进行分析，其中2017年1—12月期间仅实行常规审方干预，2018年1月制定了详细的药师干预管理标准，观察该标准实施后的2018年1—12月期间抗肿瘤药物不合理医嘱发生率的变化情况。结果52例抗肿瘤药物不合理医嘱中主要存在溶媒选择不当、药物用量不当、给药浓度不当、其他等不合理医嘱，其中溶媒选择不当最多，占48.07%;通过药师干预管理标准的制定和实施，抗肿瘤药物不合理医嘱发生率从2017年1—12月期间的0.056%下降至2018年1—12月期间的0.021%，效果明显。结论制定和实施药师干预管理标准可有效控制静配中心抗肿瘤药物不合理医嘱发生率。药师应通过对药学知识的不断积累和完善，严格把关抗肿瘤药物医嘱的合理性，以促进临床合理安全用药。
