Theoptimaltimingofpost--prostatebiopsymagnetic

ThegoaIofourstudywastoevaluatetheimpactoftheintervalbetweenprostatebiopsyandmagneticresonanceimaging(MRI)
ontheaccuracyofsimpletumorIocalization．whichisessentiaIinformationthatenablesnerve-sparingsurgery．Wealsosoughtto
determinetheoptimaltimingofapost-biopsyMRI．AtotaIof184patientswhohadundergoneMRlbeforeradicaIprostatectomyat
aninstitutionwithoutapredeterminedscheduleforMRlafteraprostatebiopsywereenrolled．ThemeanintervaIfromthebiopsyto
theMR1was30．8土18．6days．TheaccuracyoftheMRIforsimplifiedtumorIocation(right，left，bilateralandnone)was44．6％．In
thegroupwithdiscordantpathologicandMRIfindings．themostcommonreasonrecordedwasM‘RIpredictedaunilateralIesion，
butpathologyrevealedbilaterallesions’(58．3％)。followedbyM‘RlpredictednoIesion．butpatholoygrevealedthepresenceofa
Iesion’(32．0％)．Multivariableanalysisshowedthatthediscordantgrouphadashorterinterval(25．0±14．3ys38．1±20．6days，
P0．01)precedingtheMRlandahigherrateofhemorrhageasobservedbyMRI(80．4％vs54．8％。P0。01)incomparison
withtheaccordantgroup。Inreceiveroperatingcharacteristicsanalysis．theareaunderthecurveoftheMRlintervaIInaccurate
predictionofthetumorIocationwas0．707(尸0．001)．AttheMRIinterval’scuto仟of28．5days．thesensitivitywas73．2％and
thespecificitywas63．7％．WhentheMR1wasperformedwithin28days．theaccumulatedaccuracywasonly26．1％(23／88)；
however，whenitwasperformedafter28days。thereverselyaccumulatedaccuracywas61．5％(59／96)．Thesedatasupporta
waitingperiodofatleast4weeksafterabiopsybeforeperforminganMRIforthepurposesofsurgicalrefinement．
