晚期不可切除胰腺癌化疗患者精神状态及生存分析

目的调查晚期不可切除胰腺癌化疗患者的精神状态及生存情况。方法回

顾性分析该院2004年1月—2015年1月收治的159例晚期不可切除胰腺癌化疗

患者的临床资料，随访至2015年12月，随访结束时采用症状自评量表（SCL-90）

对所有患者近1个月的精神状态进行调查，对生存患者的生活质量进行评分，并

统计患者1年、2年、3年及5年的累积生存率。结果159总1年累积生存率27.0%，

2年累积生存率11.0%，3年累积生存率2.0%，5年累积生存率0.6%，总随访中

位生存期10.32个月；总介入治疗后MST8.11分。18例胰腺癌患者症状自评量

表评分结果中，除强迫及人际关系外，胰腺癌患者的躯体化、抑郁、焦虑、敌对

性、恐怖、偏执及精神病性均显著高于常模（P＜0.05）。分析患者精神状态异常

的相关因素可知，患者担心生命安全、医疗费用高、并发症等是主要影响因素。

结论晚期不可切除胰腺癌患者生存率低，患者精神状态相对较差，针对这类患

者要加强心理疏导，采取合理的综合治疗措施，以缓解患者精神压力，延长患者

生存时间。
