[Abstract]?Objective?To?investigate?the?clinical?efficacy?of?tiotropium?bromide?in?the?treatment?of?patients?with?acute?exacerbation?of?chronic?obstructive?pulmonary?disease?（AECOPD）?complicated?with?acute?respiratory?failure.?Methods?A?total?of?106?patients?with?AECOPD?complicated?with?acute?respiratory?failure?admitted?to?the?First?Peoples?Hospital?of?Huzhou?from?March?2019?to?March?2022?were?selected?and?divided?into?observation?group?and?control?group?according?to?random?number?table?method，?with?53?cases?in?each?group.?Both?groups?were?given?routine?treatment.?The?control?group?was?treated?with?noninvasive?biphasic?positive?airway?pressure?ventilation.?The?observation?group?was?treated?with?tiotropium?bromide?on?the?basis?of?control?group.?Both?groups?were?treated?for?14?days.?The?clinical?efficacy，?lung?function，?blood?gas?index，?inflammatory?factors?and?adverse?reactions?were?compared?between?two?groups.?Results?The?total?effective?rate?of?observation?group?was?significantly?higher?than?that?of?control?group?（94.34%?vs.?79.25%，?χ2=5.267，?P=0.023）.?After?treatment，?forced?expiratory?volume?in?one?second?（FEV1），?FEV1/forced?vital?capacity，?peak?expiratory?flow，?pH，?percutaneous?arterial?oxygen?saturation?and?arterial?partial?pressure?of?oxygen?in?observation?group?were?significantly?higher?than?those?in?control?group?（P0.05），?while?arterial?partial?pressure?of?carbon?dioxide，?interleukin-6，?interleukin-8，?tumor?necrosis?factor-α?and?hypersensitive?C-reaction?protein?were?lower?than?those?in?control?group?（P0.05）.?There?were?n
