医疗决策中医患双主体博弈的分析与讨论

〔摘要〕随着多元价值主义体系的构建，患者自主权的概念逐渐落地。医疗家长主义决策模式受到广泛

的质疑与抨击，进而逐渐失去主导地位，医患双方因立场及价值观差异而出现理念分歧与决策冲突，医患博

弈贯穿于医疗决策的全过程。但在剖析医疗家长主义决策模式和患者自主决策模式的优劣性后可知，盲目

贬低某种医疗决策模式并非实现患者权益最大化的最优解。医方需要充分尊重患者自主权，患方也应明确

权责对等理念，通过政策制度的优化培育医患双方的独立决策人格和理性思考能力，以平等的对话与沟通

实现医患共识。

AbstractWiththeconstructionofapluralisticvaluesystem，theconceptofpatientautonomyhas

graduallybeenestablished.Thepaternalisticmedicaldecision-makingmodelhasbeenwidelyquestionedand

criticized，graduallylosingitsdominantposition.Duetothedifferencesinpositionsandvalues，thereare

conceptualdifferencesanddecision-makingconflictsbetweendoctorsandpatients，andthedoctor-patientgame

runsthroughouttheentiremedicaldecision-makingprocess.However，afteranalyzingtheadvantagesand

disadvantagesofboththepaternalisticmedicaldecision-makingmodelandthepatientautonomousdecision-

makingmodel，itcanbefoundthatblindlydevaluinganymedicaldecision-makingmodelisnottheoptimal

solutiontomaximizepatients’rights
