460 ChineseJournalofModern OperativeSurgery，Dec．2012，Vo1．16，No．6 · 临 床 论 著 · [文章编号]1009—2188(2012)06—0460—03 剖宫产后留置尿管开放状态对排尿的影响 李华珍 (广州市番禺区第二人民医院妇产科，广州511470) [摘要] 目的 探讨剖宫产术后留置尿管间歇开放与持续开放对拔除导尿管后排尿功能 的影响。 方法 选择 192例行剖宫产术的产妇 ，随机分为两组 。实验组 96例 ，留置尿管夹 闭，间隔4h或有尿意感开放 ；对照组96例 ，留置尿管持续开放。观察拔 出尿管后第一次排尿 时间及是否需诱导排尿等情况 。 结果 实验组拔除导尿管后第一次排尿时间(2．25±0．12)h 和 自行排尿成功率为95．83％，均高于对照组第一次排尿时间 (3．1l±0．15)h和 自行排尿成功率 72．92％(P0．05)，实验组尿道刺激征出现率(2．08％)明显低于对照组(10．42％)(P0．05)。 结论 剖宫产术后留置尿管间歇开放 ，可有效减少排尿困难，预防尿潴留的发生 ，有利于产妇身 体的恢复。 [关键词]剖宫产术 ；导尿管插入术；排尿 [中图分类号]R719．8 [文献标识码]B StudyoftheUrinationafterContinuousorIntermitOpenStateoftheUrethralCatheterinPafien~ af- terCesareanDelivery L／Hua—zhen (theSecondPeople§HospitalofPanyuDistrict，Guangzhou511470， Guangdong，China) Abstract： Objective Tostudytheeffectofcontinuousflowofurineandintermittentflowofurine throughurethralcatheterin patientsaftercesarean section． M ethods 192 casesofelectivecesarean sectionwererandomlydividedintotwogroups．96patientswereintheexperimentalgroupwithindwelling catheterbeenopenedafter4-hour—clampingorbeenopenedwhenthepatientfeltlikeurination．96patients incontrolgroupwithcathetercontinuouslyopen．Theintervalbetweenthefirsturinationtimeandthecath- eterremovingwasobserved． Resul~ Thetimeofthefirsturinationandthesuccessrateofselfurination intheexperimentalgroupwas(2．25±0．12)hand95．83％ respectively，whichwashigherthanthatof thecontrolgroup，(3．1l±0．15)hand72．92％(P0．05)．Comparedwiththecontrolgroup，theex— perimentalgroupdecreasedintheincidenceofurethralirritation，10．42％vs．2．08％ (P0．05)． Conclusion Intermittentclampingofthecatheterafterthecesareansectioncanmaintainthenormalphys- iologicfunctionofthebladder，decreasethedifficultyinselfurinationandpreventurinaryretentiontohelp therecoveryofthematernalbody． Keywords：cesareansection； urinarycatheterization； urination 留置尿管是剖宫产术后常规 ，持续开放式放
