临床医学论文-长春瑞宾联合草酸铂与联合顺铂治疗晚期非小细胞肺癌的疗效比较 ????????????????? 作者：陈绍水，宁方玲，于文征 【摘要】? 目的? 比较长春瑞宾联合草酸铂与联合顺铂治疗晚期NSCLC的初步疗效及患者的耐受性。方法? 51例晚期NSCLC患者分别接受长春瑞宾+草酸铂（NL）方案或长春瑞宾+顺铂（NP）方案治疗，NL组24例，NP组27例，两组患者特征具有可比性（P＞0.05）。结果? 两组病例均可评价疗效，NL组有效率50%，其中CR1例，中位生存期11.6个月；NP组有效率48.1%，其中CR2例，中位生存期10.9个月。两组毒性反应主要为骨髓抑制，胃肠道及神经毒性。结论? NL方案和NP方案治疗晚期NSCLC疗效相似，消化道反应NP组明显，而NL组神经毒性明显，但均为轻度，属可逆性。本组资料显示NL方案与NP方案均为NSCLC的有效治疗方案，NL组耐受性较好。??? ??? 【关键词】? 晚期非小细胞肺癌；长春瑞宾；草酸铂；顺铂；化疗??? ??? This clinical trail was to compare the efficacy and toxicity of NL regimen(vinorelbine plus oxaliplatin) versus NP regimen (vinorelbine plus cisplatin) for treatment of patients with advanced non-small-cell lung cancer.Methods? The total of 51 cases were enrolled in this study，24 cases received NL regimen and 27 cases received NP regimen. The cases of two groups were well matched with baseline disease charactertics(P＞0.05).Results? The response rate of NL regimen and NP regimen was 50%(12/24)， 48.1%(13/27)respectively. The median survival was 11.6 months for NL group and 10.9 months for NP group.The major toxicity reactions were marrow depression， gastrointestinal reaction and neurotoxicity.Conclusion? The efficacy was similar to NL regimen and NP regimen in the treatment of advanced non-small cell lung cancer. Gastrointestinal reaction were higher in the NP regimen than that of NL group， but the incidence of neurotoxicity? in NL group was higher than that in NP group.In this study， NL and NP are two effective regimens for the patients with advanced NSCLC，NL regimen would be better tolerated than NP regimen.??? ??? 【Key words】? advanced non-small cell lung cancer； vinorelbine； oxaliplatin； cisplatin； chemotherapy???
