中国骨伤 2006年 12 月第 19 卷第 12 期 　Ch ina J O rthop Traum a, D ec. 2006 , Vol. 19 ,No. 12 7·57 · ·综述 · 胫骨平台骨折内固定技术的研究进展 1 2 2 2 刘俊生 ,毕大卫 ,费骏 ,郑琦 ( 1浙江中医药大学 2004 级硕士研究生 ,浙江 　杭州 　3 10053; 2浙江省中西医结合医院 ) 　　【摘要 】　胫骨平台骨折是负重关节的关节骨折 ,发病率较高 ,对康复的要求也很高 。对于胫骨平 台骨折的手术治疗 ,应首先明确其手术适应证 ,选择合理的手术入路 ,掌握正确的内固定技术 ,做到骨折 解剖复位 ,塌陷部位复位后植骨 。使用支撑钢板和松质骨螺钉做坚强内固定 ,必要时需行双髁固定 ;术 后早期 CPM 膝关节功能锻炼 ,这对于骨折愈合 、功能恢复和减少创伤性关节炎的发生十分有益 。笔者 结合近年来有关文献就其手术适应证 、手术 目的、手术入路 、切开复位及微创复位内固定技术的研究作 了回顾 、分析和展望 。 【关键词 】　胫骨骨折 ; 　骨折固定术 , 内; 　综述文献 Investiga tion progress in the trea tm en t of tib ia l p la teau fracture w ith in terna l f ixa tion techn ique　L IU J unsheng , B I D aw ei, FE I J un, ZHEN G Q i. Z hej iang Colleg e of Trad itiona l Ch inese M ed icine, H angzhou 3 10053, Z hej iang, Ch ina A BSTRACT　Tib ial p lateau fracture is in traarticu lar fractu re of weigh t loading jo in t w ith h igh incidence rate. In surgical treatm ent of tib ial p lateau fracture, the first th ing is to defin itude the op erative indication, and then choo se the app rop riate op erative app roach and p rop er in ternal fixation techn iques, so a s to obtain anatom ic reduction. For ca se s of collap se bone imp lantation shou ld be conducted after reduction. A supporting p late and cancellated bone screw can be adop ted to form a strong internal fixation, and even both ank les fixation is p er form ed in nece ssary. Po stop erative CPM functional exercise of knee joint at early atage is benefit to the fractu re healing, functional recovery and decrea sing the occurrence of traum atic arthritis.
