e0~l,医学研究 2011年4月 第9卷 第 10期 CHINESEANDFOREIGNMEDICALRESEARCH 影 礞 与 检 孚全曩 ≯|≯|叠--…|≯曩…|曩．．．．董薯譬曩誓曩薯 ≯j|||曩 |． 曩一=尊 中枢神经系统表皮样囊肿的CT诊断 伍锦平 新化县人 民医院(湖南 新化 417600) 【摘要】 目的 对于中枢神经系统表皮样囊肿的cT诊断进行分析探讨。方法 2007年 12月～2010年 12月笔者所在医院收 治的中枢神经系统表皮样囊肿患者5O例，男28例，女22例，年龄 15—66岁，平均35．25岁，使用Sytec—Synergy螺旋cT诊断。结果 36例病灶位于桥小脑角区及桥前池，8例位于鞍上、鞍旁，3例位于颞区，3例位于额叶。根据肿瘤的密度表皮样囊肿可分为两型： 典型者为均匀低密度 ，且密度低于脑脊液，或其内有少量条索样等密度，有44例，占88％；非典型者肿瘤实质呈混杂密度，瘤 内或瘤 壁有点、线状或弧形钙化，有6例，占12％。结论 随着多层螺旋cT的应用，可多方位重建成像，cT诊断表皮样囊肿有着重要价值。 【关键词】 中枢神经系统；表皮样囊肿；CT诊断 Centralnervoussystem oftheCTdiagnosisofepidermoidcystWUJin—ping．XinhuaCountyPeoplesHospital，Xinhua417600，Chi一 眦 A【bstract】 Objective ThecentralnervoussystemepidermoidcystoftheanalysisofCTdiagnosis．Methods December2007一De— cember2010admittedtoahospitalinthecentral nervoussystem epidermoidcysts，50cases，28 malesand22females，aged 15～66 years， meanagewas35．25years．Sytec—SynergyusingspiralCTdiagnosis．Results 36patientswithlesionsinthecerebellopontineangleandthe bridgebeforethepool，and8inthesaddle，thesaddlenextto3inthetemporalregion，3inthefrontal lobe．Accordingtothedensityofepider- mal cysttumorscanbedividedintotwotypes：thoseforthetypical uniform density，andthedensityislowerthaneerebrospinal fluid，orasmall amountofcord—hkeanddensity．44 cases，accountingfor88％ ；atypical bytumorparenchymawasmixeddensity，tumororaneurysm walla bit，linearorarccalcification．6cases，accountingfor12％ ．Conclusion W iththeapplicationofspiralCT，canbemulti—dimensionalrecon— struction imaging，CTdiagnosisofepidermoidcysthasasignificantvalue． 【Keywords】 Centralnervoussystem；Epidermoidcyst；CTdiagnosis 表皮样囊肿为一种 良性肿瘤，占颅脑肿瘤的0．2％一1．0％， 旁的肿瘤多呈分叶状；位于颞区的肿瘤呈卵圆形；位于额颞叶的 好发于青壮年，以脑桥小脑角区最为常见 ，其次为鞍上池、四叠 肿瘤呈椭圆形，肿瘤大小从 1．5～5．5cm不等，肿瘤边界清楚。 体、颅中窝及脑室系统亦可以发生于颅骨板障内。表皮样囊肿 2．2 肿瘤CT表现及强化特征 根据肿瘤的密度表皮样囊肿可 囊壁薄，由一层表皮组织形成，囊内由上皮碎屑、角蛋白和胆固 分为两型：典型者为均匀低密度，且密
