舒芬太尼复合利多卡因用于老年腹部手术硬膜外麻醉的临床研究 【摘要】目的： 探究在老年腹部手术硬膜外麻醉中应用舒芬太尼复合利多卡因的临床效果及安全性。方法： 选取近一年医院收治的100例择期进行腹部手术治疗老年患者，按随机双盲分组方式分为对照组和观察组，每一组例数各为50例，分别给予利多卡因加生理盐水硬膜外腔用药与舒芬太尼复合利多卡因硬膜外腔用药，比较两组患者血压、心率等的变化及麻醉效果。结果：两组对象切皮60min（T2）的心率、血压（收缩压、舒张压）等指标水平与麻醉前（T1）相比，均明显降低，但对照组患者降低的幅度较大；观察组患者达到最高感觉阻滞平面时间、感觉阻滞完善时间均显著短于对照组，而感觉阻滞持续时间显著长于对照组，且观察组患者麻醉效果3级比率明显高于对照组，以上对比差异均有统计学意义（P＜0.05）。结论： 舒芬太尼复合利多卡因应用于老年腹部手术硬膜外麻醉中，有助于维持患者的生命体征、提高麻醉效果，且起效迅速，值得临床进一步应用。 【关键词】舒芬太尼 利多卡因 老年 硬膜外麻醉 腹部手术 Clinical research of sufentanil compound lidocaine for the elderly patients in epidural anesthesia on abdominal operation [Abstracts]Objective: To discuss the clinical effect and security of sufentanil compound lidocaine for the elderly patients in epidural anesthesia on abdominal operation. Methods: 100 cases of elderly patients in epidural anesthesia on abdominal operation in our hospital were randomly divided into two groups, control group 50 cases were in epidural cavity of lidocaine and physiological saline. Observation group 50 cases were in epidural cavity of sufentanil compound lidocaine. The change of blood pressure and heart rate and anesthetic?effect of two groups were compared. Results: Compared with before anesthesia (T1), the HR, SBp and DBp of two groups at skin incision for 60 minutes(T2) were all significantly reduced, but the?declining?range?of the control group was significant. The time of highest?level of?sense?obstacle and the?time?of maximum?sensory?block?in observation group were all significantly shorter than those of the control group, but the?maintenancetime?of anesthesia in observation group was significantly longer than the control group, and the ratio of level 3 of nesthesia in observation group was significantly higher than that in control group, all the differences were statistically significant (P0.05). Conclusion: Sufentanil compound lidocaine for the elderly patients in epidural anesthesia on abdominal operation is beneficial to maintaining the patients vital signs and improving t
