Council on Academic Accreditation in Audiology and Speech-Language Pathology CAA Annual Report Addendum October 2013 INTRODUCTION: All programs must demonstrate compliance with recent changes to the Standards for Accreditation of Graduate Education Programs in Audiology and Speech-Language Pathology (Effective January 2011 and January 2013). Because the questions presented in this addendum do not appear in the on-line CAA reports that are completed via the Higher Education Data System (HES), an addendum must be completed for each professional area that is accredited (A/SLP) and submitted by the due date of the program’s report to the CAA (February 1st or August 1st) so that the CAA is able to assess the program’s compliance with these standards. Complete all of the questions in this addendum and send as an email attachment to caareports@asha.org by the submission date of your CAA report (February 1st or August 1st). ******************************************************************************************* Date: CAA File #: Institution Name: Area of Study : (SLP or AUD) NOTE: One addendum must be completed and submitted for each area of study. Standard 5.3 (Effective January 2013) The program conducts regular and ongoing assessments of program effectiveness and uses the results for continuous improvement. Implementation Language: The program must document the procedures followed in evaluating the quality, currently, and effectiveness of its graduate program and the process by which it engages in systematic self-study. The document must indicate the mechanisms used to evaluate each program component, the schedule on which the evaluations are conducted and analyzed, and the program changes and/or improvements that have resulted from assessments. The program must collect and evaluate data on its effectiveness from multiple sources (e.g., students, alumni, faculty employers, off-site supervisors or preceptors, community members, persons served).
