社区与三甲医院相互转诊共同管理模式管理社区2型糖尿病的效果 　　[摘要] 目的 探讨社区与三甲医院相互转诊共同管理模式管理社区2型糖尿病的效果，为制定本地区糖尿病防治策略提供依据。 方法 选择2013年1月～2014年1月社区内的2型糖尿病患者300例，将其随机均分为观察组和对照组。对照组采取单纯社区医师对患者教育管理；观察组采取社区与三甲医院相互转诊共同管理模式对患者进行教育管理。观察两组治疗6个月后血糖、血脂和血压等临床指标，遵医率及对糖尿病等认知水平的改善情况。 结果 糖尿病患者经社区与三甲医院相互转诊共同管理后，患者对糖尿病及其并发症的认识水平，血糖、血脂和血压等临床指标以及患者的遵医率等指标均优于对照组，管理前后比较差异有统计学意义（P0.05）。 结论 社区卫生服务中心与三甲医院相互转诊共同管理模式可有效控制2型糖尿病患者的血糖，值得进一步探讨和推广。 　　[关键词] 社区；医院；相互转诊共同管理；2型糖尿病 　　[中图分类号] R197.1 [文献标识码] A [文章编号] 1674-4721（2016）02（b）-0011-03 　　Effects of tertiary hospital-community management style on type 2 diabetes patients 　　CHEN Fang YUAN Yong-feng LIAO Mei-xia LU Si-qi CHEN Rui-zhuang 　　Huangpujie Community Health Service Center of Huangpu District in Guangzhou City，Guangzhou 510700，China 　　[Abstract] Objective To provide appropriate intervention measures and strategies of diabetes disease，and investigate the effects of tertiary hospital-community management style on type 2 diabetic patients.
