柴芍六君子汤加减对肝郁脾虚型慢性乙型肝炎免疫介导发病机制的干预研究 尹燕耀，林云华，宗亚力，万勇 （南昌市第九医院中西医结合科，江西 南昌 330002） 【基金项目】 2008年江西省卫生厅中医药科研计划（2008A108） 【摘要】 目的 柴芍六君子汤加减对肝郁脾虚型慢性乙型肝炎的疗效以及免疫介导发病机制的干预研究。方法 收集60例肝郁脾虚型慢性乙型肝炎患者，随机分为对照组和治疗组，对照组给予常规内科治疗（包括常规保肝治疗、抗病毒治疗等），其中治疗组30例，在常规内科治疗基础上予柴芍六君子汤加减的中药汤剂，观察T辅助细胞（Th1、Th2、T淋巴细胞（CD4+、CD8+）、HBV-M、HBV-DNA、肝功能等。 结果 柴芍六君子汤加减可有效治疗肝郁脾虚型慢性乙型肝炎，主要表现在改善肝功能，降低乙肝表面抗原定量水平，降低病毒载量水平有关，其作用机制可能与下调T辅助细胞（Th1、Th2）、T淋巴细胞（CD4+、CD8+）有关。结论 柴芍六君子汤加减治疗肝郁脾虚型慢性乙型肝炎，可能与其能调节患者免疫功能有关。 【关键词】 柴芍六君子汤；肝郁脾虚型；慢性乙型肝炎；免疫介导 The study of intervention by Chaishao Decoction of six noble drugs on Pathogenesis of Immune mediated of Liver stagnation and spleen deficiency type Chronic hepatitis B Yin Yan-yao, Lin Yun-hua, Zong Ya-li, Wan Yong Ninth Hospital of Nan Chang, Nanchang 330002, China Abstract Objective: To study the intervention in Chaishao Decoction of six noble drugs on Pathogenesis of Immune mediated of Liver stagnation and spleen deficiency type Chronic hepatitis B. Methods: 60 patients of Liver stagnation and spleen deficiency type Chronic hepatitis B were collected and then they were randomly divided into treatment group and control group, the control group was only received conventional Department of internal medicine treatmentincluding the conventional Treatment of hepatoprotive and antiviral,etc. The treatment group were given Chaishao Decoction of six noble drugs besides conventional Department of internal medicine treatment.The helper T cellTh1, Th2, T-LymphocytesCD4+, CD8+, HBV-M, HBV-DNA, liver function were observed . Result: Chaishao Decoction of six noble drugs was effective in the treatment of liver stagnation and spleen deficiency syndrome of chronic hepatitis B, the effection was mainly in the improvement of liver function, reducing the hepatitis B surface antigen quantitative level and viral load level.
