They are divided into M-cholinoceptor blocking drugs and N-cholinoceptor blocking drugs. §1 M-cholinoceptor blocking drugs ： Atropine and atropine like biological alkaloid( 阿托品和阿托品类生物碱) Atropine 阿托品(存在于颠茄 、曼陀罗、莨菪中) 颠茄 曼陀罗 belladonna 1、（－）M-R（阻断M-受体） 2、large dose, toxic dose: （－）N1-R , （＋）CNS 选择性： （－）M-R （－）N1-R mechanism of action gland eye smooth muscle heart CNS sweat gland汗腺、 salivary gland唾液腺 lachrymal泪腺、bronchial gland 呼吸道腺体 gastric parietal cells　胃壁细胞 Pharmacological Effects 一、glands↓： decrease the secretion 二、eye 1) mydriasis 扩瞳 atropine →（－） 瞳孔括约肌M-R →开大肌功能占 优势→瞳孔扩大 atropine→ （－）瞳 孔括约肌M-R→扩瞳 →虹膜退向外缘→前 房角间隙变小 →房水 回流↓ →眼内压? 2) increase intraocular pressure　　升高眼压 3）paralysis of accommodation 调节麻痹 Atropine：睫状肌M-R↓→睫状肌松驰 →悬韧带拉紧→晶状体扁平→屈光度?→视远物清、近物模糊 characteristics 1) 作用强度与SM紧张度成正比 对痉挛的SM作用最强 selectivity to smooth muscle： gastrointestinal tract urinary bladder膀胱、 尿道 Biliary tract 胆管、 ureter 输尿管、bronchia支气管、uterus子宫 三、smooth muscle ：relaxation 1、 HR： 1) therapeutic dose HR ? （－）突触前膜M1-R →负反馈 ? →Ach释放↑ 　　特点：a. 短时、轻度 (每分钟减少4 - 8次) 　　　　 b. 部分病人出现 四、Cardiovascular system： （一）Heart 2、A(atria)-V(ventricle) conduction（房室传导）： atropine → (一) M-R →conduction↑ 2) larger dose HR↑ （－）窦房结起搏点M2-R →解除了迷走神经对心脏 的抑制，强度与迷走神经张力水平有关。 　　因此年青人心率变化明显。 Therapeutic dose ---no significant effect 原因：多数血管缺乏胆碱能神经支配 （二） Vessel and blood pressure 血管和血压 Larger dose--- dilate cutaneous vessel(皮肤血管扩张)，warm and flush(热，潮红) 机理：不明 ，与（－）M-R无关 possible reason： 1) atropine ? secretion of sweat gland ? ?
