Guidance Notes for Completion of Qualification Questionnaire Questionnaire purpose The purpose of this questionnaire is to allow us to identify a number of suitably qualified and experienced suppliers who will be invited to submit a tender for this competition. Questionnaire layout The pass/fail section has 6 distinct parts: Part A - Bidders Details - seeks background information about the bidder; this section is not normally evaluated, however the Authority may choose not to take forward bidders that cannot provide basic company information. Part B - Questions relating to Criminal Convictions and Business Probity (mandatory and discretionary reasons for exclusion); Part C - Information Regarding the Economic or Financial Standing of the bidding organisation Part D - Insurance questions. Part E - Conflict of Interest questions Part F - Membership of trade or professional body questions The scored and weighted section has 8 distinct parts: Part G – Technical or Professional Capability Part H – Business Continuity and Disaster Recovery (Optional Section) Part I – Quality Management (Optional Section) Part J– Equal Opportunities (Optional Section) Part K– Environmental Management (Optional Section) Part L – Health and Safety (Optional Section) Part M –Construction (Optional Section) Part N– Social Care (Optional Section) Questionnaire Responses Only information provided as a direct response to the questionnaire will be evaluated. Potential suppliers should respond to the questionnaire on the basis that the contracting authority has no prior knowledge of their organisation. Information and detail which forms part of general company literature or promotional brochures etc. will not form part of the evaluation process. Marketing material should not be included. Supplementary documentation may be attached to the questionnaire where bidders have been directed to do so. Such material must be clearly marked with the name of the organisation and the ques
