不稳定骨盆骨折合并四肢多发骨折治疗中骨科损伤控制的临床应用 　　doi：10.3969/j.issn.1007-614x.2014.21.29 　　摘 要 目的：探讨骨科损伤控制（DOC）在不稳定骨盆骨折合并四肢多发骨折患者治疗中的临床应用。方法：2011年1月-2014年1月收治不稳定骨盆骨折合并四肢多发骨折患者40例，根据治疗方法的不同分为观察组和对照组，观察组采用DOC模式给予处理，对照组采用常规一期切开复位内固定处理，并比较两组治疗结局以及治疗效果。结果：观察组救治成功率明显大于对照组（95.0% VS 80.0%），骨折治疗优良率明显大于对照组（85.0% VS 60.0%），且差异有统计学意义（P0.05）。结论：DOC在不稳定骨盆骨折合并四肢多发骨折治疗中效果显著，对降低患者死亡率，提高救治效果具有重要意义。 　　关键词 不稳定骨盆骨折 四肢多发骨折 骨科损伤控制 　　Clinical application of damage orthopaedics control in the treatment of unstable pelvic fracture combined with multiple fractures of limbs 　　Zhang Jian 　　Department of Orthopedics，Qianjiang Central Hospital of Chongqing City，409000 　　Abstract Objective：To explore the clinical application of damage orthopaedics control（DOC） in the treatment of unstable pelvic fracture combined with multiple fractures of limbs.Methods：40 cases of unstable pelvic fracture patients with multiple fractures of limbs were selected from January 2011 to January 2014.They were divided into the observation group and the control group according to the different methods of treatment.The observation group were treated with DOC model.The control group were treated with regular issue of open reduction and internal fixation.The treatment outcomes and treatment effects of two groups were compared.Results：The treatment success rate of the observation group was obviously higher than that of the control group（95% VS 80%）.The excellent and good rate of fracture treatment was significantly larger than that of the control group（85% VS 60%），and the difference was statistically significant（P0.05）.Conclusion：DOC in the treatment of unstable pelvic fracture combined with multiple fractures of limbs has a significant effect.It can reduce the mortality of patients.It has an important significance on the improvement of remedy result. 　　Key words Unstable pelvic fracture；Multiple fractures of limbs；Damage orthopaedics control 　　所谓骨科损伤控制（DOC）是损伤控制（DC）在骨科的应用和发展，而DC是指在患者遭受严重创伤时，根据患者的耐受程度，采取分阶段的处理原则进行处理。近年来随着经济的发展，交
