疼痛管理在经胸乳入路腔镜甲状腺手术围术期中的应用效果
[摘要] 目的 探讨围术期疼痛管理对经胸乳入路腔镜甲状腺手术后疼痛及患者满意度的影响。 方法 选择2013年 1月~2015年8月首都医科大学宣武医院普外科收治的经胸乳入路腔镜甲状腺住院患者60例,根据入院时间分为对照组(2013年1月~2014年6月)和疼痛管理组(2014年7月~2015年8月),每组各30例。对照组按照经胸乳入路腔镜甲状腺手术日常护理原则进行常规护理,疼痛管理组按照疼痛管理方案进行护理。观察并比较两组患者疼痛评分、睡眠时间、满意度。 结果 两组患者性别、年龄、手术方式比较,差异无统计学意义(P 0.05);疼痛管理组术后当日、术后第1天疼痛评分及睡眠质量均显著优于对照组,差异均有统计学意义(P 0.05);疼痛管理组术后满意度为(97.20±2.02)分,显著高于对照组[(89.83±3.25)分],差异有高度的统计学意义(P 0.01)。 结论 对经胸乳入路腔镜甲状腺手术患者围术期进行疼痛管理,对减轻术后疼痛具有显著意义。

[Abstract] Objective To evaluate the effect of perioperative pain management on postoperative pain and satisfaction of patients receiving endoscopic thyroidectomy via chest-breast approach. Methods 60 hospitalized patients from January 2013 to August 2015 received endoscopic thyroidectomy via chest-breast approach in Department of General Surgery of Xuanwu Hospital, Capital Medical University were selected. Patients were divided into two groups based on their date of hospitalization. The patients admitted from January 2013 to July 2014 were divided into control group, patients admitted from July 2014 to August 2015 were divided into pain management group, 30 patients in each group. The patients in control group were received daily routine care designed for endoscopic thyroidectomy via chest-breast approach, while patients in pain management group were given nursing care with pain management. Pain score, sleep time, degree of satisfaction of the two group were observed and compared. Results There was no statistically significant difference between the two groups with regard to gender, age, and surgical procedures (P 0.05). The postoperative pain score and the sleep quality in the first and second day of the pain management group were better than those of the control group, the differences were statistically significant (P 0.05).
