自由体位分娩对初产妇分娩结局的影响 　　[摘要] 目的 探讨自由体位分娩对初产妇分娩结局的影响。 方法 选取2014年1月～2015年12月我院产科收治3125例初产妇作为研究对象，2015年分娩的1634例初产妇为观察组，2014年分娩的1491例初产妇为对照组。观察组实行自由体位分娩护理，对照组实行传统体位分娩护理，比较两组分娩方式及母婴并发症情况。 结果 观察组自然分娩率升高，剖宫产率、宫颈裂伤率和会阴侧切率下降，差异有统计学意义（P0.05）。 结论 自由体位分娩有利于纠正枕横位、枕后位转为枕前位，缩短产程，有利于初产妇分娩结局和减轻生殖道损伤，临床效果显著，值得推广。 　　[关键词] 自由体位；初产妇；分娩结局；母婴影响 　　[中图分类号] R248.3 [文献标识码] A [文章编号] 1674-4721（2016）05（c）-0179-03 　　Influence of labor in liberal position on delivery outcome in primipara 　　XU Xia-ying FU Chun CHEN Xing-mei 　　Department of Nursing，Maternal and Child Health Hospital of Yunfu City in Guangdong Province，Yunfu 527300，China 　　[Abstract] Objective To explore the influence of labor in liberal position on delivery outcome in primipara. Methods From January 2014 to December 2015，3125 primiparae admitted into our obstetrical department were selected as research objects.There were 1634 primiparae delivered in 2015 were selected as observation group.The rest were chosen as control group.In the observation group，labor in liberal positon during nursing was provided，while in the control group，labor in traditional position during nursing was used.The delivery mode and complications of mother and infant were compared in the two groups. Results In the observation group，spontaneous delivery rate was increased，and rates of caesarean section，cervical laceration，and lateral episiotomy were decreased with statistical differences（P0.05）. Conclusion Labor in liberal positon is beneficial to correct occipitotransverse position and occipitoposterior position into occiput anterior position and shorten stages of labor，and is beneficial for delivery outcome in primipara and alleviation of genital tract injury.The clinical effect is remarkable and is worthy of promotion. 　　[Key words] Labor in liberal positon；Primipara；Delivery outcome；Influence in mother and infant 　　自然分娩是大多数产妇都要经历的一个正常的生理过程，但该过程常常伴随不同程度的疼痛，部分产妇不能忍受而转为剖宫产，造成远期并发症的发生率增加[1]。临床上，一般对住院分娩产妇采取仰卧位，近年来国内外对于自由体位分娩的技术有了深入的研究，并广泛应用于临床[2]。自由体位分娩可增
