Methods Since January 2000 ~ June 2007, 90 elderly patients with intracapsular displaced femoral neck fractures patients were randomized controlled trial, 47 cases and internal fixation, 43 cases were treated with total hip arthroplasty were followed up for 24 to 48 months, the average (28.0 + -18.0) months. hip joint function score according to Harris score, comprehensive evaluation index: including operation time, blood loss, blood transfusion, the average length of stay, bed time, full weight bearing time, total medical costs, re-operation, complications and so on. Results of internal fixation group, the average length of stay , bed time, full weight bearing time, medical costs, and complications of reoperation was significantly higher than total hip replacement group, Harris hip score, intraoperative blood loss and blood transfusion is less than total hip replacement group, the mean operative time and total hip replacement group no significant difference. Conclusion total hip arthroplasty for intracapsular displaced femoral neck fracture of elderly better than internal fixation.
