Type1diabetes(T1D)isachronicautoimmunediseaseandcharacterizedbyabsoluteinsulinde?ciency.β-cellreplacementbyislet celltransplantationhasbeenestablishedasafeasibletreatmentoptionforT1D.Thetwomainobstaclesafterislettransplantation arealloreactiveT-cell-mediatedgraftrejectionandrecurrenceofautoimmunediabetesmellitusinrecipients.Tcellsplayacentral roleindeterminingtheoutcomeofbothautoimmuneresponsesandallograftsurvival.B7-H4,anewlyidenti?edB7homolog, playsakeyroleinmaintainingT-cellhomeostasisbyreducingT-cellproliferationandcytokineproduction.Therelationship betweenB7-H4andallograftsurvival/autoimmunityhasbeeninvestigatedrecentlyinbothislettransplantationandthenonobese diabetic (NOD) mouse models. B7-H4 protects allograft survival and generates donor-speci?c tolerance.
