Theincidenceofesophagealadenocarcinoma(EAC)hasincreasedexponentiallyinthelast3decades.Barrett’sesophagus(BE)is theonlyknownprecursorofEAC.PatientswithBEhaveagreaterthan40foldshigherriskofEACcomparedwiththegeneral population. Recent years have witnessed a revolution in the clinical and molecular research related to BE. However, several aspectsofthisconditionremaincontroversial.DataregardingthetrueprevalenceofBEhavevariedwidely.Recentstudieshave suggestedalowerincidenceofEACinnondysplasticBE(NDBE)thanpreviouslyreported.Thereispaucityofprospectivedata showing a survival bene?t of screening or surveillance for BE. Furthermore, the ever-increasing emphasis on healthcare cost containmenthascalledforreexaminationofthescreeningandsurveillancestrategiesforBE.Thereisaneedforidenti?cation ofreliableclinicalpredictorsormolecularbiomarkerstorisk-stratifypatientswhomightbene?tthemostfromscreeningor surveillanceforBE.Finally,newtherapieshaveemergedforthemanagementofdysplasticBE.Inthispaper,wehighlightthekey areasofcontroversyanduncertaintysurround
