Itisknownthatirritablebowelsyndrome(IBS)isachronicdiseaseofcyclicnaturecharacterizedbyrecurrentsymptoms.IBS patientsshouldreceive,asinitialtherapeuticapproachashortcourseoftreatmentwhich,ife?ective,hastheadditionalvalueof con?rmingthediagnosis.Long-termtreatmentshouldbereservedtodiagnosedIBSpatientswithrecurrentsymptoms.Clinical trialswithstabilizedtherapiesandnewactivetreatmentsshowedanimprovementofthesymptomsoverplacebothatisoften time-dependentbutwithhighrelapserates(around40%–50%whenstoppingtreatment).Relapseisnotalwaysimmediateafter stopping treatment and the recent data from OBIS trial with otilonium bromide or with psychotherapy, showed that due to di?erentchemico-physicalcharacteristicsofthedrugsorthepsychosomaticimpacttothediseasenotalltreatmentgavethesame relapsingrateifcomparedtoplacebo.ResultsofIBSclinicaltrialswithdi?erenttherapiestailoredtothepatientneedsindicate thatacyclictreatmenttherapyisadvisabletocounteractthenatureofthedisease. 1.NaturalHistoryofIBS werepresentinabout20%ofthedays.Themeanduration of the symptoms was about 5 days for pain and bloating and between 1 and 2 days for the other symptoms.
