Shigellosis or bacillary dysentery, an acute bloody diarrhoea, is a major public health burden in developing countries.Intheabsenceofpromptandappropriatetreatment,theinfectionisoftenfatal,particularlyinyoungmalnourished children.Here,wedescribeanewdiagnostictestforrapiddetection,instool,atthebedsideofpatients,ofShigellaflexneri2a, themostpredominantagentoftheendemicformofthedisease.Methodology/PrincipalFindings.Thetestisbasedonthe detectionofS.flexneri2alipopolysaccharide(LPS)usingserotype2a-specificmonoclonalantibodiescoupledtogoldparticles anddisplayedonone-stepimmunochromatographicdipstick.Aconcentrationaslowas20ng/mlofLPSisdetectedindistilled waterandinreconstitutedstoolsinunder15minutes.Thethresholdofdetectioncorrespondstoaconcentrationof56107 CFU/mlofS.flexneri2a,whichprovidesanunequivocalpositivereactioninthreeminutesindistilledwaterandreconstituted stools.Thespecificityis100%whentestedwithabatteryofShigellaandunrelatedstrains,inculture.WhentestedinVietnam, onclinicalsamples,thespecificityandsensitivitywere99.2and91.5%,respectively.Adecreaseofthesensitivityduringthe evaluationonstoolsampleswasobservedafterfiveweeksatroomtemperatureandwasduetomoisteningofthedipsticks causedbythehumidityoftheairduringthefifthweekoftheevaluation.Thisdrawbackisnowovercomebyimprovingthe packaging and providing dipsticks individually wrapped in waterproof bags.
